【摘要】：背景:近年來,中國藥物政策存在著藥品費(fèi)用快速增長,以及質(zhì)量、定價和提供能力等方面的問題,其解決的關(guān)鍵在于理清其中的定價和補(bǔ)償問題。方法:本文較為系統(tǒng)收集了藥物領(lǐng)域的學(xué)術(shù)文獻(xiàn)和灰色文獻(xiàn),以及中國政府的官方統(tǒng)計(jì)數(shù)據(jù),并結(jié)合中國國務(wù)院發(fā)展研究中心舉辦的研討會信息,概述了中國的藥品定價和補(bǔ)償情況。結(jié)果:通過一個融合了供應(yīng)方、代理方和需求方多方政策的框架,分析了定價與補(bǔ)償政策。在目前藥品政策的相互影響下,出現(xiàn)了藥品價格高、質(zhì)量低、不合理開藥等問題。分立的監(jiān)管環(huán)境也不利于藥品政策的改革。結(jié)論:提高藥品質(zhì)量標(biāo)準(zhǔn)、市場集中度、政府補(bǔ)貼,以質(zhì)量為導(dǎo)向的招標(biāo)、擴(kuò)大零差率政策的實(shí)施范圍、建立補(bǔ)償與合理開藥的聯(lián)動機(jī)制以及促進(jìn)衛(wèi)生技術(shù)評估與比較效能研究來改善定價和補(bǔ)償政策。另外,整合廣泛的監(jiān)管分立體制、改善透明度和打擊腐敗等問題將有助于確保制定出具有一致性的、循證的政策。
[Abstract]:Background: in recent years, China's drug policy has a rapid growth in drug costs, as well as quality, pricing and supply capacity, and the key to solve the problem is to clear up the pricing and compensation. Methods: this paper systematically collected the academic literature and grey literature in the field of drugs, as well as the official statistics of the Chinese government, and combined with the information of the seminar organized by the Development Research Center of the State Council of China. The drug pricing and compensation in China are summarized. Results: pricing and compensation policies were analyzed through a framework of supply-side, agent and demand-side policies. Under the mutual influence of the current drug policy, there are many problems such as high price, low quality and unreasonable prescription. A separate regulatory environment is also not conducive to drug policy reform. Conclusion: improve drug quality standards, market concentration, government subsidies, quality-oriented bidding, and expand the implementation of zero-margin policy. To improve the pricing and compensation policy, we should establish the linkage mechanism of compensation and rational prescription, and promote the evaluation of health technology and the research of comparative effectiveness. In addition, integrating a broad regulatory separation system, improving transparency and combating corruption will help ensure consistent, evidence-based policies.
