【摘要】：目的:促進(jìn)靜脈用藥集中調(diào)配中心(PIVAS)醫(yī)囑審核工作的規(guī)范化。方法:介紹我院PIVAS開展的醫(yī)囑審核規(guī)范化工作情況,通過比較規(guī)范化工作開展前(2016年第二季度)、后(2016年第三季度)藥師對不合理醫(yī)囑的干預(yù)率和醫(yī)師對藥師用藥建議的采納率,評價(jià)規(guī)范化工作開展的成效。結(jié)果:我院PIVAS通過規(guī)范審核標(biāo)準(zhǔn)、及時(shí)維護(hù)和更新藥品說明書、建立相關(guān)資料庫等方法開展了醫(yī)囑審核規(guī)范化工作,規(guī)范化后藥師醫(yī)囑審核標(biāo)準(zhǔn)一致、依據(jù)充分,審核效率提高、審核結(jié)果更準(zhǔn)確。與規(guī)范化工作開展前比較,規(guī)范化工作開展后干預(yù)率下降[1.41%(308/21 776)vs.0.74%(168/22 750)]、采納率上升(95.1%vs.98.8%)。結(jié)論:我院開展的醫(yī)囑審核規(guī)范化工作提高了臨床醫(yī)師開具醫(yī)囑的合理性及對藥師用藥建議的接受程度。
[Abstract]:Objective: to promote the standardization of (PIVAS) doctor's order audit in the central intravenous drug dispensing center. Methods: to introduce the standard work of doctor's order audit carried out by PIVAS in our hospital, and to compare the intervention rate of pharmacist to unreasonable doctor's order and the acceptance rate of doctor's advice to pharmacist's medication before (the second quarter of 2016) and after (the third quarter of 2016). To evaluate the effectiveness of standardization work. Results: by standardizing the auditing standard, maintaining and updating the drug instruction in time, and establishing the relevant database, the PIVAS carried out the standardization of the examination of the doctor's orders, and the standard of the examination of the doctor's orders was the same after the standardization, and the basis was sufficient. Improve the efficiency of audit, audit results more accurate. Compared with that before standardization, the intervention rate decreased by 1.4 1% (30 / 21 776) vs.0.74% (168 / 22 750), and the adoption rate increased (95. 1% vs 98. 8%). Conclusion: the standardization of doctor's order audit in our hospital has improved the reasonableness of prescribing doctor's order and the acceptance of pharmacist's suggestion of medication.
