【摘要】：目的:了解南通市7家醫(yī)院胸外科圍術期預防性使用抗菌藥物的情況,為患者臨床合理用藥提供參考。方法:選取南通市7家"三級"醫(yī)院2015-2016年行胸外科手術的出院患者6652例進行回顧性分析。統(tǒng)計患者的性別、年齡、住院時間、術前用藥情況、用藥時機、切口分類、手術時間、藥物名稱、用法用量、給藥途徑、聯合用藥、預后情況等。根據整治活動相關規(guī)定和圍術期抗菌藥物使用原則對抗菌藥物預防性使用的合理性進行評價。結果:6652例患者中,Ⅰ類切口共有2761例,Ⅱ類切口共有3891例,全部使用了抗菌藥物。初次預防用藥時機在術前0.5～2h的共有5282例,占79.40%;術前2h以上或術后給藥的有1370例,占20.60%。預防用藥48h以內的有1070例,占16.09%;超過48h的有5582例,占83.91%。單用一種抗菌藥物的有5702例,兩聯用藥的有950例。共有6342例患者不合理使用抗菌藥物,占95.34%。使用的抗菌藥物共涉及八大類15個品種,使用最多的為β-內酰胺類/β-內酰胺酶抑制劑、氟喹諾酮類和氧頭孢烯類。結論:專項整治后,南通市7家醫(yī)院胸外科預防性使用抗菌藥物仍存在諸多不合理現象,部分指標超出國家規(guī)定,仍需進一步管理和改進。
