【摘要】：目的:為處方質(zhì)量的持續(xù)改進提供參考。方法:隨機抽取我院門急診2014年每月某天的小包裝中藥飲片處方,共5 151張,按照國家和我院制定的有關(guān)中藥飲片處方的管理規(guī)定,對所選處方的合理性以及不合理處方類別和百分比進行點評。結(jié)果:5 151張?zhí)幏街?合理處方為3 237張,占抽查處方的62.84%;不合理處方為1 914張,占抽查處方的37.16%。各類不合理處方數(shù)及其構(gòu)成比依次為:大處方(包括處方金額、劑數(shù)、藥味數(shù)、貴重藥質(zhì)量超過規(guī)定等)1 040張,占54.34%;缺中醫(yī)診斷652張,占34.06%;無煎服法或煎服法不準(zhǔn)確108張,占5.64%;急診處方超3劑56張,占2.93%;調(diào)劑復(fù)核無雙簽字39張,占2.04%;缺臨床診斷14張,占0.73%;毒性藥品超劑量或使用"十八反"藥對未注明原因和再次簽名4張,占0.21%;診斷與用藥不符1張,占0.05%。結(jié)論:我院門急診小包裝中藥飲片處方總的合格率還不理想,持續(xù)改進工作仍需加強。
[Abstract]:Objective: to provide reference for the continuous improvement of the quality of the prescription. Methods: a total of 5151 pieces of traditional Chinese medicine decoction pieces were randomly selected from the emergency department of our hospital in 2014. According to the regulations on the prescription of Chinese herbal medicine prepared by the state and our hospital, the rationality of the selected prescriptions as well as the categories and percentages of the irrational prescriptions were reviewed. Fruit: of the 5151 prescriptions, the rational prescription was 3237, accounting for 62.84% of the selected prescription. The irrational prescription was 1914. The number of irrational prescriptions and its constituent ratio of 37.16%., which accounted for 1040, 54.34% of the large prescription (including the amount of the prescription, the number of agents, the number of drugs, the quality of the valuable medicine, and so on), accounted for 54.34%, and 652 of traditional Chinese medicine, accounting for 34.06%. No decocting method or decocting method was inaccurate 108, accounting for 5.64%; emergency prescription over 3 doses 56 sheets, accounting for 2.93%, no double signature 39, 2.04%, 14 clinical diagnosis, 0.73%, toxic drugs or eighteen counter drugs, excluding reasons and 4 resignatures, accounting for 0.21%; diagnosis and drug use did not conform to 3, 0.05%. conclusion: our hospital is urgent. The total eligibility rate of Chinese medicine decoction pieces in small package is not satisfactory, and the continuous improvement work still needs to be strengthened.
