【摘要】：目的:為人血白蛋白臨床合理使用提供參考。方法:回顧性調(diào)查我院2014年1-12月使用人血白蛋白的住院患者病例,統(tǒng)計患者一般資料、用藥前血清白蛋白水平、用藥量、費用、用藥原因等信息,并進行分析。結(jié)果:人血白蛋白使用人數(shù)最多的8個科室共8 760例患者納入研究,占全院使用人血白蛋白住院患者的64%。其中,男性5 244例(占59.9%),女性3 516例(占40.1%),患者年齡主要集中在30~60歲(占52.6%)。白蛋白使用人數(shù)最多的科室為普通外科(2 588例)和心血管外科(2 348例)。以國內(nèi)說明書作為參照標準時,該院有8 210例(約93.7%)患者符合適應(yīng)證,低蛋白血癥為最常見的用藥指征(占35.6%)。重癥監(jiān)護治療病房人均使用量最高(152.5 g),普通外科總用藥金額最高(13 334 396.4元),傳染科人均使用療程最長(10.7 d)。結(jié)論:我院存在人血白蛋白不合理使用情況,科室間使用情況存在差異。應(yīng)加強對人血白蛋白使用的監(jiān)督,建立合理的用藥評估體系,以促進用藥規(guī)范,實現(xiàn)醫(yī)療資源合理分配。
[Abstract]:Objective: to provide reference for the rational use of human serum albumin (HSA). Methods: the data of patients with human serum albumin (HSA) in our hospital from January to December 2014 were retrospectively investigated and analyzed. Results: 8 760 patients were included in the study in 8 departments, which accounted for 64% of the inpatients who used human serum albumin (HSA) in the whole hospital. There were 5 244 males (59.9%) and 3 516 females (40.1%). The age of the patients was mainly 30 ~ 60 years old (52.6%). The most common use of albumin was in general surgery (2 588 cases) and cardiovascular surgery (2 348 cases). When the domestic specification was taken as the reference standard, there were 8 210 patients (93. 7%) in this hospital. Hypoproteinemia was the most common indication of drug use (35. 6%). The per capita usage in intensive care unit was 152.5 g / g, the total sum of total drug used in general surgery was 13 334 396.4 yuan, and the longest course of treatment was 10.7 d in infectious department. Conclusion: unreasonable use of human serum albumin exists in our hospital, and there is difference between departments. The supervision of the use of human serum albumin (HSA) and the establishment of a rational drug use evaluation system should be strengthened in order to promote the standardization of drug use and realize the rational distribution of medical resources.
