【摘要】：目的:探討3種蛇毒類凝血酶制劑在外科手術的止血有效性,并進行經濟學分析,為臨床選擇最佳止血方案提供依據(jù)。方法:回顧性抽取2015年1月-2016年1月在某院骨科腰椎手術及四肢骨折內固定術選用單一止血藥注射用白眉蛇毒血凝酶(邦亭)、蛇毒血凝酶注射液(速樂涓)和注射用矛頭蛇毒血凝酶(巴曲亭)的手術病例303例進行臨床療效和經濟學研究。結果:邦亭、速樂涓和巴曲亭在腰椎手術及四肢骨折內固定術的止血臨床有效率分別為76.31%,81.08%,75.00%和72.13%,76.92%,73.33%,兩兩比較均無統(tǒng)計學差異(P0.05)。不良反應發(fā)生率比較,差異性無統(tǒng)計學意義(P0.05)。采用最小成本分析法進行經濟學分析,3組患者除蛇毒血凝酶制劑費用和處理不良反應費用外的所有費用基本相等。邦亭組、速樂涓組和巴曲亭組及處理不良反應的費用在腰椎手術及四肢骨折內固定術分別為384.7,451,1 189.1,443,506.1,1 223.4元,兩兩比較結果顯示,邦亭組與速樂涓組比較均無顯著性差異(P0.05),巴曲亭組分別與邦亭組與速樂涓組比較均存在顯著性差異(P0.05)。結論:注射用白眉蛇毒血凝酶(邦亭)和蛇毒血凝酶注射液(速樂涓)用于腰椎手術及四肢骨折內固定術術中止血的經濟性明顯優(yōu)于注射用矛頭蛇毒血凝酶(巴曲亭)。
[Abstract]:Objective: to investigate the hemostatic efficacy of three snake venom thrombin preparations in surgical procedures and to provide evidence for clinical selection of the best hemostatic regimen. Methods: from January 2015 to January 2016, hemagglutinase (Bang Ting Pu, snake venom hemagglutinin injection) was used in lumbar vertebra surgery and internal fixation of extremities fracture for single hemostatic injection. A clinical and economic study was carried out in 303 patients treated with hemagglutinin from the venom of the spearhead snake. Results: the hemostasis rates of Bangting, Sulejuan and Baxuting in lumbar vertebrae operation and internal fixation of extremities fracture were 76.31% and 72.13%, respectively, and 72.13%, respectively. There was no significant difference between the two groups (P 0.05). There was no significant difference in the incidence of adverse reactions (P 0.05). The least cost analysis method was used to analyze the three groups of patients with the same cost except snake venom hemagglutinin preparation and the cost of adverse reactions. The cost of treatment of side effects in the Bongting group, Sulejuan group and bactritin group were 384.7451U 1 189.1443506.1 yuan (RMB) in lumbar vertebrae operation and internal fixation of extremities fracture, respectively. The results showed that There was no significant difference between Bongting group and Sulejuan group (P 0.05), but there was significant difference between Baqu Ting group and Bongting group and Sulejuan group (P 0.05). Conclusion: the cost of hemostasis of Baimei snake venom hemagglutinin and snake venom hemagglutinase injection in lumbar vertebrae operation and internal fixation of extremities fracture is better than that of bartrexine injection.
