小兒腹股溝疝修補術(shù)圍手術(shù)期預(yù)防用藥干預(yù)管理與效果評價

【摘要】:目的:評價小兒腹股溝疝修補術(shù)圍手術(shù)期預(yù)防用藥的干預(yù)管理效果,探索適合小兒腹外疝修補術(shù)預(yù)防用藥的干預(yù)模式。方法:選取2011年手術(shù)病例作為非干預(yù)組,回顧性分析圍手術(shù)期預(yù)防用藥存在的主要問題,將2012-2015年手術(shù)病例作為干預(yù)組,對圍手術(shù)期抗菌藥物預(yù)防使用率、平均預(yù)防用藥天數(shù)、平均預(yù)防用藥費用、平均住院天數(shù)、手術(shù)部位感染率等指標(biāo)進行重點監(jiān)控和干預(yù)管理。結(jié)果:經(jīng)過4年干預(yù)管理,小兒腹股溝疝修補術(shù)圍手術(shù)期抗菌藥物預(yù)防使用率由干預(yù)前的100%下降為干預(yù)后的0%;平均預(yù)防用藥天數(shù)由干預(yù)前的≤1 d下降為干預(yù)后的0d;平均預(yù)防用藥費用由干預(yù)前的37.66元下降為干預(yù)后的0元;平均住院天數(shù)均≤1 d;干預(yù)前后手術(shù)部位感染率無顯著性差異(P0.05)。結(jié)論:小兒腹股溝疝修補術(shù)圍手術(shù)期預(yù)防用藥干預(yù)措施具有可行性和有效性,對安全、有效、經(jīng)濟、合理地使用抗菌藥物起到了積極的促進作用,為醫(yī)院設(shè)計單病種臨床路徑和降低單病種費用提供了參考依據(jù),也為進一步規(guī)范其它清潔手術(shù)預(yù)防用藥提供了可操作性的干預(yù)模式。
[Abstract]:Objective: to evaluate the intervention and management effect of preventive medication in perioperative period of inguinal hernia repair in children, and to explore the intervention mode suitable for prevention of external hernia repair in children. Methods: the patients with inguinal hernia repair in 2011 were selected as non-intervention group. The main problems of prophylactic drug use in perioperative period were analyzed retrospectively. The cases of operation from 2012 to 2015 were used as intervention group. The utilization rate of antimicrobial agents, the average days of prophylactic drug use, the average cost of prophylactic drug use, the average days of hospitalization, and the duration of hospitalization in the perioperative period were analyzed retrospectively. The infection rate of surgical site was monitored and managed. Results: after 4 years of intervention management, The utilization rate of antimicrobial agents in perioperative period of inguinal hernia repair in children decreased from 100% before intervention to 0 after intervention, the average days of prophylactic medication decreased from 鈮,
