【摘要】：目的:為降低臨床抗腫瘤藥不合理用藥發(fā)生率提供參考。方法:調(diào)取我院靜脈藥物配置中心超然管理系統(tǒng)中2012年9月-2014年2月抗腫瘤藥醫(yī)囑,對不合理醫(yī)囑進(jìn)行分析和總結(jié)。結(jié)果:在審核的51 204條醫(yī)囑中,不合理用藥為201條,占0.39%;不合理用藥主要表現(xiàn)在溶劑選擇不當(dāng)(42.79%)、給藥劑量過大或過小(17.91%)、溶劑體積不合理(32.33%)、給藥途徑錯誤(3.98%)、給藥順序不合理(2.49%)以及其他方面(0.50%),醫(yī)囑修改率分別為97.67%、100%、95.38%、100%、80.00%及100%。結(jié)論:通過定期分析、總結(jié),以醫(yī)師聯(lián)系單的形式及不合理醫(yī)囑的匯總形式與病房溝通,可以加強(qiáng)對抗腫瘤藥應(yīng)用的監(jiān)督,保障患者用藥安全。
[Abstract]:Objective: to provide a reference for reducing the incidence of irrational use of antitumor drugs. Methods: from September 2012 to February 2014, the antitumor doctors' orders were obtained from the transcendental management system of the intravenous Drug allocation Center in our hospital. Analysis and summary of unreasonable medical orders. Results: of the 51,204 medical orders examined, 201 were irrational drug use. The irrational use of drugs was mainly manifested in improper choice of solvent (42.79), excessive or too small dose of medicine (17.91U), unreasonable volume of solvent (32.331U), wrong way of administration (3.98b), irrational order of administration (2.49th) and other aspects. The revised rate of doctor's orders was 97.67100%, 95.38%, 80.00% and 100000, respectively. Conclusion: through regular analysis, In conclusion, to communicate with the ward in the form of doctor contact list and unreasonable doctor's advice can strengthen the supervision of the application of antitumor drugs and ensure the safety of drug use by patients.
