【摘要】：目的:推動省級藥事質(zhì)量控制中心對重點(diǎn)監(jiān)控藥品的有效監(jiān)管,促進(jìn)合理用藥。方法:介紹四川省藥事質(zhì)量控制中心二片區(qū)通過督促轄區(qū)醫(yī)療機(jī)構(gòu)建立重點(diǎn)監(jiān)控藥品監(jiān)管制度、制定3類重點(diǎn)監(jiān)控藥品處方點(diǎn)評指南、開展基線數(shù)據(jù)調(diào)查、現(xiàn)場督查醫(yī)療機(jī)構(gòu)重點(diǎn)監(jiān)控藥品的實(shí)際使用情況等措施,發(fā)揮對3類重點(diǎn)監(jiān)控藥品[中藥注射劑、輔助用藥、質(zhì)子泵抑制劑(PPIs)]使用的指導(dǎo)、督促作用;通過收集各分中心向省質(zhì)控中心的工作報告,統(tǒng)計醫(yī)療機(jī)構(gòu)建立監(jiān)管制度的情況;通過隨機(jī)抽樣統(tǒng)計各分中心轄區(qū)醫(yī)療機(jī)構(gòu)重點(diǎn)監(jiān)控藥品的合理使用率、使用量、使用金額等數(shù)據(jù),對監(jiān)管效果進(jìn)行參考性評價。結(jié)果:截至2016年4月,92.26%的醫(yī)療機(jī)構(gòu)知曉省衛(wèi)計委的相關(guān)文件要求,84.25%已建立本院的監(jiān)管制度;2016年3月已完成3類重點(diǎn)監(jiān)控藥品處方(醫(yī)囑)點(diǎn)評指南的制定;省內(nèi)18家三級甲等綜合醫(yī)療機(jī)構(gòu)3類重點(diǎn)監(jiān)控藥品2016年5月不合理使用率與1月比較,中藥注射劑下降12.35%(35.41%vs.47.76%)、輔助用藥下降9.64%(20.95%vs.30.59%)、PPIs下降7.44%(40.49%vs.47.93%);2016年4月平均銷售金額與1月比較,分別下降12.07%、13.30%、9.49%;55.02%的醫(yī)療機(jī)構(gòu)已啟動基線數(shù)據(jù)資料收集工作。結(jié)論:省級藥事質(zhì)量控制中心在重點(diǎn)監(jiān)控藥品管理制度的建設(shè)上可發(fā)揮重要作用,可指導(dǎo)和督促醫(yī)療機(jī)構(gòu)建立監(jiān)管制度,評估分析各級醫(yī)療機(jī)構(gòu)監(jiān)管情況,指出存在的問題,進(jìn)而促進(jìn)重點(diǎn)監(jiān)控藥品有效監(jiān)管。
[Abstract]:Objective: to promote the effective supervision of key controlled drugs by provincial drug quality control center. Methods: by urging the medical institutions in Sichuan Province to establish the supervision and control system of the key controlled drugs, the author introduced three kinds of guidelines for the evaluation of the prescription of the controlled drugs. Carry out baseline data investigation, on-site supervision of medical institutions, monitoring of the actual use of drugs and other measures, give play to the three types of key monitoring drugs. [Traditional Chinese medicine injection, adjuvant drug, proton pump inhibitor (PPIsa)] the guidance of the use of the role of supervision; Through collecting the work reports of the sub-centers to the provincial quality control center, statistics on the establishment of supervision system in medical institutions; Through random sampling statistics of the medical institutions in each sub-center area to monitor the rational use of drugs, usage, use amount of data and other data, the regulatory effect of reference evaluation. Results: up to April 2016. 92.26% of the medical institutions know the relevant documents of the provincial health and family planning commission require that 84.25% have established their own supervision system; In March 2016, the guidelines for the evaluation of three major types of controlled drug prescriptions (doctor's orders) were completed. There were 18 Grade 3A comprehensive medical institutions in the province. The irrational utilization rate of 3 kinds of key controlled drugs in May 2016 was compared with that in January. The Chinese medicine injection dropped 12.35% and 35.41% vs.47.76%, and the auxiliary medicine dropped 9.64% 20.95vs.30.59). PPIs decreased 7.44 and 40.49, including vs.47.93; Compared with January, the average sales amount in April 2016 decreased by 12.07 and 13.30% respectively. 55.02% medical institutions have started baseline data collection. Conclusion: provincial drug quality control center can play an important role in the construction of key drug management system. It can guide and urge medical institutions to establish supervision system, evaluate and analyze the supervision of medical institutions at all levels, point out the existing problems, and then promote the effective supervision of key monitoring drugs.
