【摘要】：藥品價(jià)格虛高是當(dāng)前我國(guó)醫(yī)改需直面的主要問(wèn)題�；舅幬�"零差價(jià)"管制政策試圖通過(guò)政府招標(biāo)價(jià)、醫(yī)院采購(gòu)價(jià)和藥品零售價(jià)三價(jià)合一的方式,來(lái)切斷醫(yī)藥之間的利益鏈條,達(dá)到終結(jié)"以藥養(yǎng)醫(yī)"的政策目標(biāo)�；谀繕�(biāo)測(cè)評(píng)法,對(duì)基本藥物"零差價(jià)"進(jìn)行政策效果評(píng)估的結(jié)論為:藥價(jià)虛高主要由于行政性公立醫(yī)院壟斷所致,"零差價(jià)"管制政策并不能完全有效消除"以藥養(yǎng)醫(yī)"體制,并會(huì)伴隨一些管制內(nèi)生性問(wèn)題。政策實(shí)施會(huì)使醫(yī)院的議價(jià)能力降低,醫(yī)生拿回扣等商業(yè)賄賂行為向基層醫(yī)院擴(kuò)展,醫(yī)院或醫(yī)生合法賺取的"顯性租金"演變成暗箱操作的"隱形租金"。這就導(dǎo)致政策目標(biāo)出現(xiàn)了醫(yī)院虧損面擴(kuò)大和藥費(fèi)不降反升的偏差。建議尊重市場(chǎng)規(guī)律,發(fā)揮醫(yī)院和醫(yī)生主體作用,合理利用醫(yī)保支付方式來(lái)控制費(fèi)用。
[Abstract]:The virtual high of drug price is the main problem that needs to be faced directly in the medical reform of our country at present. The "zero difference" control policy of essential drugs tries to combine the government bidding price, the hospital purchase price and the drug retail price. To cut off the chain of interests between medicine, to achieve the end of the "medical care with medicine" policy objectives. Based on the objective evaluation method. The conclusion of the policy evaluation on the "zero difference" of essential drugs is that the deficiency of drug price is mainly caused by the monopoly of administrative public hospitals, and the "zero difference" control policy can not completely eliminate the system of "raising medicine with drugs". The implementation of the policy will reduce the bargaining power of the hospital, doctors will take kickbacks and other commercial bribery to expand to the grass-roots hospitals. The "explicit rent" legally earned by a hospital or doctor has evolved into a "hidden rent" operating in a dark box. This has led to a deviation in the policy objective of expanding the loss side of the hospital and increasing the drug cost instead of falling. It is suggested that the market rule be respected. Give play to the main role of hospitals and doctors, reasonable use of medical insurance payment to control costs.
