【摘要】：目的:減少我院門診藥房相似藥品調(diào)劑差錯(cuò),實(shí)現(xiàn)門診藥品溯源。方法:在對(duì)醫(yī)院信息系統(tǒng)(HIS)藥品字典后臺(tái)數(shù)據(jù)庫(kù)中加入藥品電子監(jiān)管碼字段的基礎(chǔ)上,改進(jìn)我院門診藥房處方調(diào)劑流程,在二次審核發(fā)藥操作前增加藥品電子監(jiān)管碼掃碼審核操作,并據(jù)此實(shí)現(xiàn)藥品終端溯源。通過對(duì)比改進(jìn)前、后12個(gè)月的調(diào)劑外差率、盤點(diǎn)中發(fā)現(xiàn)的與相似藥品調(diào)劑差錯(cuò)有關(guān)的"特殊異常數(shù)據(jù)"占比、患者候藥時(shí)間等指標(biāo),評(píng)價(jià)改進(jìn)前后門診藥房處方調(diào)劑質(zhì)量。結(jié)果:增加藥品電子監(jiān)管碼審核操作后,相似藥品引起的調(diào)劑外差由22例降低為1例(占總處方數(shù)比例分別為0.019 7‰、0.010 3‰),"特殊異常數(shù)據(jù)"數(shù)量由836條下降為436條(占總異常數(shù)據(jù)比例分別為31.5%、16.8%),患者平均候藥時(shí)間未見明顯延長(zhǎng)(分別為10.85、10.88 min),并實(shí)現(xiàn)了大部分門診藥品從藥房到患者之間的溯源查詢。結(jié)論:利用藥品電子監(jiān)管碼可減少門診藥房相似藥品調(diào)劑外差,有利于藥品溯源鏈的建立、提升藥學(xué)服務(wù)質(zhì)量。
[Abstract]:Objective: to reduce the errors of dispensing similar drugs in outpatient pharmacy of our hospital. Methods: on the basis of adding the field of electronic drug supervision code to the backstage database of drug dictionary of hospital information system (HIS), we improved the dispensing flow of prescription in outpatient pharmacy of our hospital. The electronic regulatory code scan operation was added before the second audit and the source of the drug terminal was realized. After 12 months of comparison and improvement, the rate of external adjustment was obtained. The proportion of "special abnormal data" related to similar drug dispensing errors and the waiting time of patients were found in the inventory. To evaluate the quality of prescription dispensing in outpatient pharmacy before and after improvement. Results: after the examination operation of electronic supervision code of drugs was increased.
