骨水泥載萬(wàn)古霉素超聲低頻脈沖型治療的起始時(shí)間對(duì)藥動(dòng)藥效產(chǎn)生的影響分析

【摘要】:目的:研究低頻脈沖超聲治療的起始時(shí)間對(duì)載萬(wàn)古霉素骨水泥的藥物動(dòng)力學(xué)、藥效釋放以及抗菌活性的影響。方法:選擇50只健康大鼠,在大鼠髖關(guān)節(jié)均植入載萬(wàn)古霉素的骨水泥以及標(biāo)準(zhǔn)金黃色葡萄球菌(ATCC13565,中國(guó)生物鑒定研究所),隨后將50只大鼠隨機(jī)分成觀察組、對(duì)照組,觀察組在植入后12 h進(jìn)行低頻超聲治療,對(duì)照組在植入后30 min進(jìn)行低頻超聲治療,兩組均治療12 h后取4只大鼠測(cè)定髖關(guān)節(jié)內(nèi)的滲血量,其余大鼠測(cè)定髖關(guān)節(jié)內(nèi)的藥物濃度,計(jì)算藥物動(dòng)力及藥效參數(shù),在植入48 h后計(jì)數(shù)髖關(guān)節(jié)腔內(nèi)的活菌量并進(jìn)行組間比較。結(jié)果:觀察組、對(duì)照組均在植入3 h后達(dá)到藥物濃度高峰,對(duì)照組3 h后藥物濃度呈明顯的下降態(tài)勢(shì),并在14 h后下降到MIC以下,觀察組在3 h后也呈下降態(tài)勢(shì),但在12 h采用低頻超聲治療后,藥物濃度再次上升,并一直持續(xù)在MIC以上至第34小時(shí);觀察組Cmax/MIC(5.58±1.86)明顯低于對(duì)照組的(14.81±2.41),差異具有統(tǒng)計(jì)學(xué)意義(P0.05);觀察組Tmic(35.08±5.98)h明顯高于對(duì)照組的(13.49±1.49)h,差異具有統(tǒng)計(jì)學(xué)意義(P0.05);對(duì)照組菌落數(shù)、滲血量明顯高于觀察組(P0.05)。結(jié)論:與植入后立即進(jìn)行超聲治療相比,在第12小時(shí)進(jìn)行治療能夠有效提高血液的Tmic,提高載萬(wàn)古霉素骨水泥的抗菌作用。
[Abstract]:Objective: to study the effect of the initial time of low-frequency pulsed ultrasound on pharmacokinetics, pharmacokinetics, release and antibacterial activity of bone cement loaded with vancomycin. Methods: 50 healthy rats were selected. Bone cement loaded with vancomycin and standard staphylococcus aureus ATCC 13565 were implanted into the hip joints of the rats, and then 50 rats were randomly divided into observation group. The control group was treated with low-frequency ultrasound 12 hours after implantation, while the control group was treated with low-frequency ultrasound 30 min after implantation. After 12 hours of treatment, 4 rats in both groups were taken to determine the amount of blood oozing in the hip joint, and the other rats measured the drug concentration in the hip joint, and calculated the pharmacokinetic and pharmacodynamic parameters. After 48 hours of implantation, the live bacteria in the hip joint were counted and compared among the groups. Results: in the observation group and the control group, the concentration of the drug reached the peak 3 h after implantation. The drug concentration in the control group decreased significantly after 3 h, and decreased below MIC after 14 h, and decreased in the observation group after 3 h, but after 12 h treatment with low frequency ultrasound. The drug concentration increased again and lasted from above to 34 hours after MIC. Cmax/MIC(5.58 鹵1.86 in the observation group was significantly lower than that in the control group (14.81 鹵2.41), and the difference was statistically significant (P 0.05). The Tmic(35.08 of the observation group was significantly higher than that of the control group (13.49 鹵1.49 h), and the difference was statistically significant (P 0.05). The number of colony in the control group was significantly higher than that in the observation group (P 0.05). Conclusion: compared with the ultrasound treatment immediately after implantation, the Tmic of blood can be effectively increased after 12 hours of treatment. The antibacterial effect of vancomycin loaded bone cement was improved.
