【摘要】：目的:為我國醫(yī)院藥學的進一步發(fā)展提供參考。方法:總結浙江地區(qū)18家三級醫(yī)院、8家�？漆t(yī)院、6家三級中醫(yī)院和中西醫(yī)結合醫(yī)院共32家醫(yī)療機構的創(chuàng)新藥學服務實踐方法。結果與結論:該地區(qū)醫(yī)療機構通過建設麻醉科衛(wèi)星藥房、開設藥學服務門診(包括開展老年綜合評估多學科門診藥學服務、由藥師管理的抗凝咨詢門診、由臨床藥師管理的內分泌咨詢門診、藥學綜合咨詢門診、藥學單元模式的門診藥學服務)、創(chuàng)立多學科協(xié)作下的藥事查房模式、開展基于藥學科研的臨床藥學服務(包括治療藥物監(jiān)測、基因檢測)和基于循證的藥學服務模式等創(chuàng)新藥學服務模式,確保了患者用藥安全,拓展了醫(yī)療服務空間,充實了醫(yī)院藥學服務內涵,可為其他地區(qū)醫(yī)療機構有效開展藥學服務提供參考。
[Abstract]:Objective: to provide a reference for the further development of hospital pharmacy in China. Innovative methods of pharmaceutical service in 32 medical institutions in 6 tertiary Chinese medicine hospitals and integrated traditional Chinese and western medicine hospitals. Results & conclusion: satellite pharmacy of anesthesiology department was constructed in this area. Setting up pharmacological outpatient service (including multi-disciplinary outpatient pharmacy service for comprehensive evaluation of the elderly, anticoagulant consultation clinic managed by pharmacist, endocrine consultation clinic managed by clinical pharmacist, and comprehensive pharmaceutical consultation clinic). The pharmacological unit model of outpatient pharmacy services, the establishment of multidisciplinary collaborative drug rounds model, the development of pharmacological research based clinical pharmaceutical services (including therapeutic drug monitoring). Gene testing) and the innovative pharmaceutical care model based on evidence ensured the safety of drug use, expanded the medical service space and enriched the connotation of hospital pharmaceutical service. It can provide reference for other medical institutions to carry out pharmaceutical service effectively.
