【摘要】：目的:自20世紀(jì)60年代以來(lái),心血管置入式電子設(shè)備(CIED,the cardiovascular implantable electronic device)成為目前心血管內(nèi)科治療相關(guān)疾病的常規(guī)設(shè)備之一,CIED相關(guān)感染是(CIED-I,the cardiovascular implantable electronic device related infection)術(shù)后嚴(yán)重的并發(fā)癥。因此該論文的主要研究目的是:為了研究CIED-I相關(guān)危險(xiǎn)因素,總結(jié)有效的預(yù)防措施,為今后臨床實(shí)踐提供借鑒。方法與結(jié)果:通過回顧性調(diào)查2009年元月1日起6年內(nèi)在該醫(yī)院心內(nèi)科心導(dǎo)管室行CIED置入或更換的患者,經(jīng)嚴(yán)格按照納入排除標(biāo)準(zhǔn)共納入CIED相關(guān)574例手術(shù)人次,并統(tǒng)計(jì)了每例發(fā)生感染患者的基本特征、基礎(chǔ)疾病、術(shù)中及術(shù)后用藥、術(shù)后并發(fā)癥等信息,所得統(tǒng)計(jì)結(jié)果通過logistic回歸分析,逐步回歸納入研究變量,通過Wald檢驗(yàn),0.05為檢驗(yàn)水準(zhǔn),共篩選出3個(gè)暴露因素納入logistic回歸模型,并解釋統(tǒng)計(jì)結(jié)果:CIED-I的保護(hù)因素為于術(shù)中應(yīng)用抗生素,危險(xiǎn)因素為應(yīng)用抗栓藥物、糖尿病。結(jié)論:該項(xiàng)回顧性研究通過數(shù)據(jù)統(tǒng)計(jì)分析認(rèn)為該中心CIED-I早期感染的發(fā)生率為1.91%,抗生素的圍手術(shù)期內(nèi)應(yīng)用可以預(yù)防感染的發(fā)生,合并糖尿病、術(shù)中應(yīng)用抗栓藥物的患者更易發(fā)生CIED-I,并通過結(jié)合既往文獻(xiàn)探討了CIED-I的預(yù)防及治療方法。
