全關(guān)節(jié)置換術(shù)在進(jìn)展型骨關(guān)節(jié)炎中的作用

【摘要】:背景:鑒于越來越多的骨關(guān)節(jié)炎患者選擇了全膝關(guān)節(jié)置換術(shù)。有部分研究表明,因?yàn)榛颊吣挲g較大或術(shù)前準(zhǔn)備不完善,全膝關(guān)節(jié)置換術(shù)患者的術(shù)前癥狀緩解不盡如人意并存在功能活動低于預(yù)期的情況,這些情況吸引了部分學(xué)者的注意,為解決這一問題,他們將注意力集中在了特定的手術(shù)技術(shù)和假體設(shè)計(jì)上,但這兩者并沒有完美的解決這一問題。為了避免引起最常見的導(dǎo)致早期手術(shù)翻修,假體感染和假體不穩(wěn)定等情況,確;颊咝g(shù)后癥狀的緩解和延長遠(yuǎn)期假體生存率,特此提出此項(xiàng)研究。目的:評價(jià)膝關(guān)節(jié)置換術(shù)治療晚期膝關(guān)節(jié)骨性關(guān)節(jié)炎的效果。研究方式:回顧性研究。方法:對昆明醫(yī)科大學(xué)第一附屬醫(yī)院確診為膝關(guān)節(jié)骨性關(guān)節(jié)炎的40例病案進(jìn)行研究。研究對象主要是影像學(xué)確診的晚期膝關(guān)節(jié)骨性關(guān)節(jié)炎患者,這些患者在2015年1月1日至2017年1月1日期間進(jìn)行了全膝關(guān)節(jié)置換術(shù),手術(shù)指征是膝關(guān)節(jié)嚴(yán)重疼痛。結(jié)果:患者年齡(平均值±標(biāo)準(zhǔn)差):66.35±8.29,女性人數(shù)為70%,男性人數(shù)為30%。52.5%的患者行左側(cè)TKA,右側(cè)的為45.5%;85%患者診斷為膝關(guān)節(jié)骨性關(guān)節(jié)炎4期,15%診斷為3期,術(shù)前和術(shù)后疼痛評分采用前述方法進(jìn)行統(tǒng)計(jì)學(xué)分析,P值為0.001((P0.05);前后位穩(wěn)定性的數(shù)據(jù)P值0.001(0.05);活動范圍:P值0.001(0.05)(表5和圖4)伸直受限方面:P值0.001(即0.05)。術(shù)后隨訪12個月的患者數(shù)目平均值±SD:6.6842±4.42283。術(shù)后并發(fā)癥17例,DVT 10例,肺炎10例,肺炎和DVT 5例,其余67.5%無并發(fā)癥。結(jié)論:通過本實(shí)驗(yàn)發(fā)現(xiàn),在膝關(guān)節(jié)骨關(guān)節(jié)炎3期和4期的患者中,全膝關(guān)節(jié)置換術(shù)的患者在疼痛嚴(yán)重程度(用疼痛量表測量),運(yùn)動范圍,前后位穩(wěn)定性和伸直受限程度上得到改善,這些結(jié)果具有統(tǒng)計(jì)學(xué)意義。
[Abstract]:Background: in view of the increasing number of patients with osteoarthritis, total knee arthroplasty has been chosen. Some studies have shown that patients with total knee arthroplasty have less than satisfactory preoperative symptoms and lower than expected functional activities due to their older age or inadequate preoperative preparation, which has attracted some scholars' attention. To solve this problem, they focused on specific surgical techniques and prosthesis design, but both did not solve the problem perfectly. In order to avoid the most common causes of early surgical revision, prosthetic infection and prosthetic instability, to ensure the relief of postoperative symptoms and prolong the long-term survival rate of prosthesis, this study is proposed. Objective: to evaluate the efficacy of knee arthroplasty in the treatment of advanced osteoarthritis of the knee. Research methods: retrospective study. Methods: 40 cases of knee osteoarthritis diagnosed in the first affiliated Hospital of Kunming Medical University were studied. The subjects were radiographically diagnosed patients with advanced knee osteoarthritis who underwent total knee arthroplasty from January 1, 2015 to January 1, 2017, indicating severe knee pain. Results: the age (mean 鹵standard deviation) of the patients was 66.35 鹵8.29, the number of women was 70, the number of men was 30.52.5%, the right side of TKA, was 45.5%; 85% of the patients were diagnosed as stage 4 of knee osteoarthritis and 15% of the patients with stage 3. The preoperative and postoperative pain scores were statistically analyzed by the above-mentioned method (P = 0.001). The range of activity: P value 0.001 (Table 5 and Fig. 4): P value 0.001 (P = 0.001). The average number of patients followed up for 12 months was 鹵SD:6.6842 鹵4.42 283. There were 17 cases of postoperative complications, 10 cases of DVT, 10 cases of pneumonia, 5 cases of pneumonia and DVT. The other 67.5% had no complications. Conclusion: in the patients with stage 3 and stage 4 of knee osteoarthritis, the patients undergoing total knee arthroplasty had severe pain (measured by pain scale) and exercise range. The anterior and posterior position stability and the degree of extension limitation were improved, and the results were statistically significant.
