2型糖尿病患者血尿酸水平與胰島功能相關(guān)性的研究

【摘要】:目的:本課題通過觀察120例患者的血尿酸水平與胰島功能各評估指標(biāo)胰島素分泌指數(shù)(HOMA-β)、胰島素抵抗指數(shù)(HOMA-IR)、早期胰島素分泌功能指數(shù)(△I30/G30)、胰島素曲線下面積(AUCI)、胰島素曲線下面積/葡萄糖曲線下面積(AUCI/AUCG),期望發(fā)現(xiàn)血尿酸水平與胰島β細(xì)胞分泌功能及抵抗程度的關(guān)系。以期為2型糖尿病(T2DM),尤其是合并高尿酸血癥(HUA)的T2DM患者的早期治療提供相關(guān)理論依據(jù)。方法:根據(jù)入排標(biāo)準(zhǔn),收集2015年11月—2016年11月于我科住院的單純T2DM患者60例,T2DM合并HUA患者60例作為觀察對象。研究對象均知情同意。診斷均符合HUA診斷標(biāo)準(zhǔn)及1999年WHO 2型糖尿病診斷標(biāo)準(zhǔn)。記錄病患的一般資料及生化檢驗指標(biāo)。根據(jù)公式計算HOMA-β、HOMA-IR、△I30/G30、AUCI、AUCI/AUCG。統(tǒng)計學(xué)分析應(yīng)用SPSS19.0完成。結(jié)果:1.單純T2DM組與T2DM合并HUA組兩組間胰島分泌功能評估指標(biāo)HOMA-β、△I30/G30、AUCI及胰島素抵抗評估指標(biāo)HOMA-IR、AUCI/AUCG差異均有統(tǒng)計學(xué)意義(P0.05);2.胰島功能與UA水平相關(guān)性分析:患者HOMA-β、△I30/G30、AUCI,HOMA-IR、AUCI/AUCG與UA水平存在正相關(guān)性(r=0.610,r=0.277,r=0.197,r=0.496,r=0.220,p0.05)。3.兩組間BMI、FPG、PBG、BUN、Scr、胱抑素、視黃醇有統(tǒng)計學(xué)差異(P0.05)。結(jié)論:1.與單純T2DM患者相比,合并HUA患者IR程度更高;2.與單純T2DM患者相比,合并HUA患者胰島β細(xì)胞分泌功能升高;A(chǔ)胰島素分泌、第一時相胰島素分泌、第二時相胰島素分泌功能隨著血尿酸水平的升高而增加;3.UA是影響胰島β細(xì)胞分泌功能及IR的重要因素之一。
[Abstract]:Objective: to observe the serum uric acid level and insulin secretion index (HOMA- 尾), insulin resistance index (HOMA-IR), early insulin secretion index (I30/G30), area under insulin curve (AUCI), area under glucose curve (AUCI/AUCG). It is expected to find the relationship between serum uric acid level and islet 尾 cell secretion and resistance. In order to provide theoretical basis for early treatment of type 2 diabetes mellitus (T2DM), especially T2DM patients with hyperuricemia (HUA). Methods: according to the admission criteria, 60 patients with simple T2DM and 60 patients with T2DM complicated with HUA were collected from November 2015 to November 2016. All the subjects had informed consent. The diagnosis was in accordance with HUA diagnostic criteria and WHO type 2 diabetes mellitus diagnostic criteria in 1999.
