【摘要】：正病例簡介:患者王某,女,68歲,主因"多關(guān)節(jié)腫痛4年,雙足麻木6月"入院。4年前出現(xiàn)周身多關(guān)節(jié)腫痛,以雙手小關(guān)節(jié)為著,晨僵約半小時,伴雙側(cè)膝關(guān)節(jié)腫脹,查類風(fēng)濕因子686 U/ml(0~20),抗環(huán)瓜氨酸肽抗體250 U/ml,抗核抗體1∶1 000,ENA系列陰性,C反應(yīng)蛋白16.9 mg/L(0~8),診斷為類風(fēng)濕關(guān)節(jié)炎,先后予甲氨蝶呤10 mg qw聯(lián)合來氟米特20 mg qd,來氟米特20 mg qd聯(lián)合雷公藤20 mg tid后加用柳氮磺吡啶0.5 g bid,并間斷輸入云克治
