腸旁路術(shù)后GK大鼠消化吸收功能變化情況及其對(duì)血糖血脂的影響

【摘要】:【目的】研究腸旁路術(shù)(Duodenal-jejunal bypass,DJB)后非肥胖型2型糖尿病(Goto-Kakizaki,GK)大鼠消化吸收功能變化情況及其對(duì)血糖、血脂的影響。同時(shí)和改良膽胰轉(zhuǎn)流術(shù)(New Billiopancreatic diversion,NBPD)組及假手術(shù)組進(jìn)行對(duì)比研究,觀察三組GK大鼠葡萄糖、淀粉、蛋清灌胃后血糖變化及正常飲食后,血糖血脂的變化情況,探討外科手術(shù)治療2型糖尿可能的機(jī)制。【方法】1.15周齡雄性GK大鼠30只,依據(jù)體重完全隨機(jī)分為三組,分別行十二指腸-空腸轉(zhuǎn)流術(shù)(DJB組10只)、改良膽胰轉(zhuǎn)流術(shù)(NBPD組10只)、假手術(shù)組(SHAM組10只)。2.術(shù)前1周,術(shù)后1、2、4、6周檢測(cè)空腹體重;術(shù)前2周,術(shù)后2、4、6周檢測(cè)空腹血糖(FPG)和隨機(jī)血糖(RBG)。3.術(shù)前2周術(shù)后2、4、6周,做口服糖耐量試驗(yàn)(OGTT)。4.術(shù)前2周及術(shù)后6周,測(cè)空腹及餐后2h血脂。5.分別于術(shù)后第42、43天,使用50%葡萄糖和紅薯淀粉按照8kcal/kg的熱量進(jìn)行灌胃,尾靜脈采血測(cè)0、30、60、120、180min血糖,并計(jì)算曲線下面積(AUC)。6.術(shù)后第46天,使用蛋清按照8kcal/kg的熱量進(jìn)行灌胃,然后測(cè)0,30,60,120,180min血糖值,計(jì)算出血糖曲線下面積(AUC)。【結(jié)果】1.術(shù)前,三組大鼠空腹體重差異均無(wú)統(tǒng)計(jì)學(xué)意義。術(shù)后1周,三組大鼠體重低于術(shù)前,差異有統(tǒng)計(jì)學(xué)意義。術(shù)后2、4、6周三組大鼠空腹體重差異無(wú)統(tǒng)計(jì)學(xué)意義。術(shù)前2周,及術(shù)后2、4、6周三組GK大鼠空腹血糖之間差異均無(wú)統(tǒng)計(jì)學(xué)意義。術(shù)后2、4、6周,DJB組和NBPD組隨機(jī)血糖低于假手術(shù)組,術(shù)后2、6周DJB組、NBPD組和術(shù)前相比,差異有統(tǒng)計(jì)學(xué)意義,在術(shù)后6周,NBPD組和假手術(shù)組差異有統(tǒng)計(jì)學(xué)意義。2.術(shù)前2周,三組大鼠之間OGTT血糖曲線下面積差異無(wú)統(tǒng)計(jì)學(xué)意義,術(shù)后2、4、6周,DJB組和NBPD組OGTT血糖曲線下面積明顯小于假手術(shù)組,在術(shù)后4、6周,NBPD組OGTT血糖曲線下面積和假手術(shù)之間差異有統(tǒng)計(jì)學(xué)意義。3.術(shù)前2周,三組大鼠之間空腹及餐后2h膽固醇,甘油三酯,游離脂肪酸無(wú)差異。在術(shù)后6周,DJB組、NBPD組空腹膽固醇、游離脂肪酸和假手術(shù)相比,差異具有統(tǒng)計(jì)學(xué)意義,DJB組餐后2h膽固醇、游離脂肪酸和假手術(shù)組及術(shù)前相比,差異有統(tǒng)計(jì)學(xué)意義。4.使用淀粉和葡萄糖灌胃后,DJB和NBPD組血糖曲線下面積都明顯低于假手術(shù)組,兩種物質(zhì)灌胃后NBPD組血糖曲線下面積和假手術(shù)相比,差異均統(tǒng)計(jì)學(xué)意義。DJB組和NBPD組淀粉灌胃后30、60、120min血糖值及血糖曲線下面積均小于葡萄糖灌胃的值。5.蛋清灌胃后,三組大鼠血糖值稍微波動(dòng),血糖曲線下面積差異無(wú)統(tǒng)計(jì)學(xué)意義!窘Y(jié)論】1.腸旁路術(shù)(DJB)、改良膽胰轉(zhuǎn)流術(shù)(NBPD)都能改善GK大鼠血糖。2.DJB術(shù)和NBPD術(shù)后,都改善GK大鼠血脂。3.DJB術(shù)和NBPD術(shù)后,GK大鼠消化吸收功能的變化,和膽胰液的轉(zhuǎn)流有關(guān),這可能是改善血糖血脂的主要機(jī)制之一。
[Abstract]:[Objective] To study the changes of digestive and digestive function of non-obese type 2 diabetes (Goto-Kakizaki, GK) rats after intestinal bypass (DJB) and its effect on blood sugar and blood lipid. [Methods] Thirty-one (15)-week-old male GK rats were randomly divided into three groups according to their weight, and were divided into three groups according to their weight, and were divided into three groups according to the weight of body weight. The fasting body weight was detected in the first, second, 4th and 6th week after operation. The fasting blood glucose (FPG) and the random blood sugar (RBG) were measured at 2,4 and 6 weeks after the operation.3. The difference of the area under the OGTT blood glucose curve of the two weeks,6 weeks,6 weeks, DJB group and NBPD group was significantly lower than that of the sham operation group, and the difference between the area under the OGTT blood glucose curve and the false operation of the NBPD group was statistically significant at 4,6 and 6 weeks after the operation. [Conclusion] 1. Enteral bypass (DJB) and modified BPD can improve the blood sugar of GK rats.2. DJB and NBPD improve the blood lipid of GK rats.3. The changes of the absorption function of GK rats after DJB and NBPD are related to the bypass of the bile and pancreatic juice, which may be one of the main mechanisms to improve blood glucose and blood lipid.
