高尿酸血癥患者血清胱抑素C與腎小球?yàn)V過(guò)率的相關(guān)性研究

【摘要】:目的通過(guò)檢測(cè)高尿酸血癥(HUA)患者血清胱抑素-C(Cys-C)水平與GFR的相關(guān)性,探討其在HUA性腎損傷早期診斷中的價(jià)值。方法 選取無(wú)癥狀HUA患者(sHUA)60例、痛風(fēng)(Gout)患者50例和正常對(duì)照組(NC)60名。改良MDRD公式計(jì)算eGFR,并測(cè)定各組血清Cys-C、β2微球蛋白(β2-MG)、Scr、SUA、TG、高敏C反應(yīng)蛋白(hsC-RP)等生化指標(biāo)。分析血清Cys-C、β2-MG與eGFR、Scr等指標(biāo)的關(guān)系。結(jié)果 Gout組血清Cys-C、β2-MG、Scr、SUA、hsC-RP、TG、BMI高于NC組(P0.05或P0.01),eGFR、HDL-C低于NC組(P0.05),Cys-C、hsC-RP、BMI、β_2-MG高于sHUA組(P0.05)。sHUA組Cys-C、SUA、TG高于NC組,HDL-C低于NC組。血清Cys-C與eGFR呈負(fù)相關(guān)(r=-0.457,P=0.003),與Scr、年齡、SUA呈正相關(guān)(r=0.472、0.325、0.171,P=0.000、0.001、0.05)。多元線性回歸分析顯示,eGFR、Scr、SUA、年齡是血清Cys-C水平升高的危險(xiǎn)因素。結(jié)論血清Cys-C能反映GFR的變化,是HUA性腎損傷早期診斷的良好血清標(biāo)志物。
[Abstract]:Objective to investigate the correlation between serum cystatin C (Cys-C) and GFR in patients with hyperuricemia (HUA) and to explore its value in early diagnosis of HUA renal injury. Methods 60 patients with asymptomatic HUA (sHUA), 50 patients with gout (Gout) and 60 normal controls were selected. The serum Cys-C, 尾 2 microglobulin (尾 2-MG) and Scr,SUA,TG, Gao Min C reactive protein (hsC-RP) were calculated by modified MDRD formula. The relationship between serum Cys-C, 尾 2-MG and eGFR,Scr was analyzed. Results in Gout group, the serum Cys-C, 尾 2-MGG-ScrSG-SUAHS-C-RPGN was higher than that of NC group (P0.05 or P0.01), eGFR,HDL-C was lower than that of NC group (P0.05), Cys-C,hsC-RP,BMI, was higher than that of NC group (P0.05). 尾 _ 2-MG was higher than sHUA group (P0.05) Cys-C,SUA,TG in). SHUA group was higher than that in NC group, HDL-C was lower than NC group. There was a negative correlation between serum Cys-C and eGFR (r = -0.457), and a positive correlation between serum Cys-C and Scr, age and SUA (r = 0.472 ~ 0.325 ~ 0.171 ~ 0.0000.000 / 0. 05). Multiple linear regression analysis showed that the age of eGFR,Scr,SUA, was a risk factor for the increase of serum Cys-C level. Conclusion Serum Cys-C can reflect the changes of GFR and is a good serum marker for early diagnosis of HUA renal injury.
