【摘要】：目的分析原發(fā)性甲狀旁腺功能亢進(jìn)癥(primary hyperparathyroidism,PHPT)病人伴發(fā)甲狀腺病變的發(fā)病情況,探討PHPT與甲狀腺疾病的關(guān)系。方法回顧分析84例PHPT病人資料,統(tǒng)計(jì)PHPT合并甲狀腺病變的詳細(xì)發(fā)病情況及PHPT對(duì)病人甲狀腺功能的影響,使用SPSS統(tǒng)計(jì)軟件分析PHPT與甲狀腺疾病的相關(guān)性以及PHPT對(duì)病人甲狀腺功能的影響。結(jié)果本組PHPT病例合并甲狀腺疾病主要為結(jié)節(jié)性甲狀腺腫(62/74),其中單發(fā)結(jié)節(jié)性甲狀腺腫25例(33.8%),多發(fā)結(jié)節(jié)為37例(50.0%),甲狀腺毮樣癌(medullary thyroid carcinoma,MTC)病人12例(16.2%)。PHPT病人總T4(total tetraiodothyronine,TT4)及游離T4(free tetraiodothyronine,FT4)濃度較正常成年人降低(P0.05)。TT4及FT4均與甲狀旁腺激素(serum parathyroid hormone,PTH)濃度呈負(fù)相關(guān)(P0.05)。結(jié)論 PHPT病人甲狀腺疾病高發(fā),且TT4及FT4濃度較正常成年人降低,其原因可能與高PTH相關(guān)。
[Abstract]:Objective to investigate the relationship between PHPT and thyroid disease in patients with primary hyperparathyroidism (primary hyperparathyroidism,PHPT). Methods the data of 84 patients with PHPT were analyzed retrospectively. The incidence of PHPT complicated with thyroid lesions and the effect of PHPT on thyroid function were analyzed retrospectively. The correlation between PHPT and thyroid disease and the effect of PHPT on thyroid function were analyzed by SPSS software. Results PHPT patients with thyroid diseases were mainly nodular goiter (62 / 74), including 25 cases (33.8%) with single nodular goiter, 37 cases (50.0%) with multiple nodule, and (medullary thyroid carcinoma, with thyroid carcinoma. The concentrations of total T4 (total tetraiodothyronine,TT4) and free T4 (free tetraiodothyronine,FT4) in 12 patients with MTC (16.2%). PHPT) were lower than those in normal adults (P0.05). TT4 and FT4 were associated with the parathyroid hormone (serum parathyroid hormone,. The concentration of PTH was negatively correlated (P0.05). Conclusion there is a high incidence of thyroid disease in patients with PHPT, and the concentrations of TT4 and FT4 are lower than those of normal adults, which may be related to high PTH.
