【摘要】：目的系統(tǒng)評(píng)價(jià)強(qiáng)骨膠囊與維生素D治療骨質(zhì)疏松性骨折的有效性及安全性。以期為臨床骨質(zhì)疏松性骨折患者的藥物選擇提供參考。方法計(jì)算機(jī)檢索Pub Med、Web of science、Cochrane圖書館、相關(guān)期刊論文(CNKI)、中國(guó)生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(kù)(CBM)、中文科技期刊全文數(shù)據(jù)庫(kù)(VIP)、萬(wàn)方數(shù)據(jù)庫(kù)。搜集強(qiáng)骨膠囊與維生素D用于治療骨質(zhì)疏松性骨折的隨機(jī)對(duì)照試驗(yàn)(RCT),檢索年限均為建庫(kù)以來(lái)至2017年1月27日。由兩位研究者獨(dú)立篩選文獻(xiàn)、提取資料,并評(píng)價(jià)納入研究的偏倚風(fēng)險(xiǎn)后,采用Rev Man 5.2軟件進(jìn)行Meta分析。結(jié)果最終納入3個(gè)RCT,包括510例患者。Meta分析結(jié)果顯示:強(qiáng)骨膠囊組患者的骨折愈合時(shí)間顯著低于維生素D組,兩組比較差異有統(tǒng)計(jì)學(xué)意義[MD=-3.68,95%CI(-4.60,-2.76),P0.00001]。強(qiáng)骨膠囊組的治療后股骨頸骨密度(bone mineral density,BMD)高于維生素D組,且差異均具有統(tǒng)計(jì)學(xué)意義[MD=0.03,95%CI(0.01,0.04),P=0.0002]。強(qiáng)骨膠囊組患者輕度不良反應(yīng)發(fā)生數(shù)顯著高于維生素D組,兩組比較差異有統(tǒng)計(jì)學(xué)意義[OR=23.89,95%CI(4.56,125.11),P=0.0002]。結(jié)論強(qiáng)骨膠囊可以減少骨質(zhì)疏松性骨折患者的骨折愈合時(shí)間,增強(qiáng)骨密度,但相較于維生素D,仍需注意胃腸道的不良反應(yīng)。
[Abstract]:Objective to evaluate the efficacy and safety of Qianggu capsule and vitamin D in the treatment of osteoporotic fracture. In order to provide a reference for the choice of drugs in patients with osteoporotic fracture. Methods Pub Med,Web of science,Cochrane library, full text database of Chinese periodicals, (CNKI), database of Chinese biomedical literature, (CBM), database of full text of Chinese sci-tech periodicals, (VIP), Wanfang database, were searched by computer. The (RCT), search time of the randomized controlled trial of collecting Qianggu capsule and vitamin D for the treatment of osteoporotic fracture was from the establishment of the bank to January 27, 2017. The two researchers independently sifted through the literature, extracted the data and evaluated the bias risks involved in the study. The Meta analysis was performed with Rev Man 5.2 software. Results the results of Meta analysis showed that the fracture healing time in Qianggu capsule group was significantly lower than that in vitamin D group, and the difference between the two groups was statistically significant [MD=-3.68,95%CI (-4.60), " -2.76), P0.00001]. The bone mineral density (bone mineral density,BMD) of femoral neck in Qianggu capsule group was higher than that in vitamin D group, and the difference was statistically significant [MD=0.03,95%CI (0.01 ~ 0.04), P ~ (0.0002)]. The incidence of mild adverse reactions in Qianggu capsule group was significantly higher than that in vitamin D group, and the difference between the two groups was statistically significant [OR=23.89,95%CI (4.56125.11), P _ (0.0002)]. Conclusion Qianggu capsule can reduce fracture healing time and enhance bone density in patients with osteoporotic fracture, but the adverse reactions of gastrointestinal tract should be paid more attention to compared with vitamin D.
