【摘要】：類風(fēng)濕性關(guān)節(jié)炎(rheumatoid arthritis,RA)是一種病因尚不明確,以多發(fā)性和對(duì)稱性關(guān)節(jié)病變?yōu)橹鞯姆翘禺愋匝装Y,主要病理變化是關(guān)節(jié)滑膜的慢性炎癥和增生,滑膜邊緣增生形成血管翳,關(guān)節(jié)軟骨和軟骨下骨被破壞、吸收。RA在治療方面雖然有了很大的進(jìn)步,但是目前仍然沒有完全根治的治療辦法。治療RA的傳統(tǒng)藥物主要有三大類:傳統(tǒng)的非甾體類抗炎藥、糖皮質(zhì)激素和生物制劑,這些藥物可以改善癥狀,減緩病情的進(jìn)展,但不能根治RA,且藥物的不良反應(yīng)也較多。因此,開發(fā)安全有效的新型藥物將是治療RA的主要研究方向之一。通過我們前期細(xì)胞實(shí)驗(yàn)的研究結(jié)果表明,小分子SYK抑制化合物HL131078對(duì)TNF-α誘導(dǎo)的類風(fēng)濕關(guān)節(jié)炎成纖維滑膜細(xì)胞的增殖具有良好的抑制作用,且能夠抑制成纖維滑膜細(xì)胞分泌炎性介質(zhì)IL-6和MMP-3。目的本課題擬從動(dòng)物實(shí)驗(yàn)水平,觀察小分子SYK抑制化合物HL131078對(duì)CIA小鼠膝關(guān)節(jié)滑膜增生、軟骨和骨質(zhì)破壞及分泌IL-6和MMP-3的影響,探討此化合物對(duì)RA的治療效果,為今后開發(fā)治療RA的新型藥物提供依據(jù)。研究方法采用CIA小鼠模型,將小鼠分為空白組、模型組、陽性組、實(shí)驗(yàn)組(高、低濃度),分別給予灌胃治療,通過觀察小鼠爪子腫脹程度、MIC-CT分析、小鼠膝關(guān)節(jié)組織石蠟切片HE染色、番紅O-固綠染色、免疫組化來評(píng)價(jià)此小分子化合物對(duì)CIA小鼠膝關(guān)節(jié)滑膜的增生、軟骨和骨質(zhì)的破壞和分泌IL-6、MMP-3的影響。統(tǒng)計(jì)學(xué)處理數(shù)據(jù)處理采用SPSS19.0軟件,結(jié)果以?x±s表示,組間比較采用兩樣本t檢驗(yàn),P0.05表示差異有統(tǒng)計(jì)學(xué)差異。結(jié)果1、經(jīng)過灌胃治療后的實(shí)驗(yàn)組小鼠后爪腫脹程度較模型組相比有明顯消褪,小鼠后肢平均關(guān)節(jié)炎指數(shù)出現(xiàn)明顯下降趨勢(shì)。2、MIC-CT結(jié)果顯示,模型組小鼠膝關(guān)節(jié)骨質(zhì)破壞嚴(yán)重,實(shí)驗(yàn)組小鼠膝關(guān)節(jié)關(guān)節(jié)表面未遭到明顯侵蝕,骨質(zhì)仍維持較好完整性。3、在關(guān)節(jié)組織病理學(xué)的結(jié)果中,實(shí)驗(yàn)組小鼠膝關(guān)節(jié)組織切片HE染色及番紅O-固綠染色可見,滑膜細(xì)胞增生不明顯,關(guān)節(jié)腔內(nèi)僅有少量炎性細(xì)胞浸潤(rùn),膝關(guān)節(jié)表面軟骨侵蝕不嚴(yán)重,與模型組相比,病理學(xué)改變明顯減輕。4、在免疫組化結(jié)果中,實(shí)驗(yàn)組小鼠膝關(guān)節(jié)中IL-6和MMP3的分泌量明顯減少。結(jié)論小分子SYK抑制化合物HL131078可以有效緩解CIA小鼠膝關(guān)節(jié)中滑膜的增生,減輕關(guān)節(jié)表面軟骨侵蝕及骨質(zhì)破壞情況,減少關(guān)節(jié)腔中IL-6、MMP-3的分泌及炎性細(xì)胞的浸潤(rùn)。
[Abstract]:Rheumatoid arthritis (rheumatoid arthritis,RA) is a non-specific inflammation with multiple and symmetrical joint lesions. The main pathological changes are chronic inflammation and hyperplasia of synovial membrane and pannus of synovial edge hyperplasia. Articular cartilage and subchondral bone are destroyed and absorbed. Although RA has made great progress in treatment, there is still no complete cure. There are three main categories of traditional drugs for RA: traditional non-steroidal anti-inflammatory drugs, glucocorticoids and biological agents. These drugs can improve symptoms and slow down the progress of the disease, but can not cure RA, and the adverse drug reactions are more. Therefore, the development of safe and effective new drugs will be one of the main research directions in the treatment of RA. The results of our previous cell experiments showed that the small molecule SYK inhibitor HL131078 had a good inhibitory effect on the proliferation of fibroblasts from rheumatoid arthritis induced by TNF- 偽. It also inhibited the secretion of inflammatory mediators IL-6 and MMP-3. by fibroblast synoviocytes. Objective to observe the effects of small molecule SYK inhibitor HL131078 on synovial hyperplasia of knee joint, cartilage and bone destruction and secretion of IL-6 and MMP-3 in CIA mice at the animal experimental level, and to explore the therapeutic effect of the compound on RA. To provide the basis for the development of new drugs for the treatment of RA in the future. Methods CIA mice were divided into three groups: blank group, model group, positive group, experimental group (high and low concentration), which were treated by gastric perfusion respectively. The degree of paw swelling in mice was observed and analyzed by MIC-CT. The effects of this small compound on synovium proliferation, cartilage and bone destruction and secretion of IL-6,MMP-3 in CIA mice were evaluated by using paraffin sections of mouse knee joint with HE staining, phenanthrene O-solid green staining and immunohistochemistry. SPSS19.0 software was used to process the statistical data. The results were expressed as? X 鹵s, and two samples t test were used for the comparison between groups. The difference was statistically significant (P0.05). Results 1. The swelling degree of hind paw in the experimental group was obviously decreased compared with that in the model group, and the average arthritis index of the hind limb of the experimental group was obviously decreased. 2 the results of MIC-CT showed that the swelling degree of the hind paw of the experimental group was significantly decreased than that of the model group. In the model group, the knee joint bone destruction was serious, the surface of the knee joint in the experimental group was not obviously eroded, and the bone still maintained a good integrity. 3 in the results of joint histopathology, In the experimental group, the synovial cell proliferation was not obvious, only a few inflammatory cells infiltrated in the articular cavity, and the cartilage erosion on the surface of the knee joint was not serious, compared with the model group. The pathological changes were obviously alleviated. 4. In the immunohistochemical results, the secretion of IL-6 and MMP3 in the knee joint of the experimental group was significantly decreased. Conclusion small molecule SYK inhibitor HL131078 can effectively relieve synovium proliferation, cartilage erosion and bone destruction, decrease IL-6,MMP-3 secretion and inflammatory cell infiltration in the articular cavity of CIA mice.
