足三里穴位注射治療糖尿病下肢神經(jīng)病變的臨床觀察

【摘要】：目的：本課題研究用足三里穴位注射維生素B1、維生素B6的方法治療糖尿病下肢神經(jīng)病變患者,并觀察患者治療前后下肢神經(jīng)傳導(dǎo)速度及癥狀、體征的改善情況,以證實(shí)足三里穴位注射治療糖尿病下肢周圍神經(jīng)病變的療效,為治療糖尿病下肢神經(jīng)病變提供可靠的方法。方法：選取符合納入標(biāo)準(zhǔn)的糖尿病下肢神經(jīng)病變患者120例,采用隨機(jī)數(shù)字表法,將患者隨機(jī)分為A、B、C、D四組各30例,A組給予基礎(chǔ)治療,B組給予基礎(chǔ)治療和甲鈷胺肌肉注射,C組給予基礎(chǔ)治療和維生素B1、B6肌肉注射,D組給予基礎(chǔ)治療和足三里穴位注射維生素B 1、B6。四組患者均治療2周。觀察中醫(yī)證候積分、雙下肢脛神經(jīng)傳導(dǎo)速度(NCV)改變情況,收集數(shù)據(jù),經(jīng)統(tǒng)計(jì)分析得出結(jié)果。結(jié)果：1.中醫(yī)證候積分比較中,治療后組間比較可知,D組與其他三組比較,差異均具有統(tǒng)計(jì)學(xué)意義(P0.05),B組與A、C兩組比較,均具有統(tǒng)計(jì)學(xué)意義(P0.05)；組內(nèi)比較可知,B組與D組治療前后比較,均具有統(tǒng)計(jì)學(xué)意義(P0.05)。2.有效率比較,足三里穴位注射維生素B1、B6組患者總有效率為86.67%,與其他組比較,P0.05,均具有統(tǒng)計(jì)學(xué)意義3.神經(jīng)傳導(dǎo)速度比較,治療后組間比較可知,D組與其他三組比較,差異均具有統(tǒng)計(jì)學(xué)意義(P0.05),B組和C組分別與A組比較,均具有統(tǒng)計(jì)學(xué)意義(P0.05),B組和C組比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。組內(nèi)治療前后比較,B組、C組和D組治療前后比較,均具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論：1.足三里穴位注射維生素B1、B6能夠明顯改善糖尿病下肢神經(jīng)病變血瘀脈阻型患者的癥狀和體征。2.足三里穴位注射維生素B1、B6能夠明顯提高糖尿病下肢神經(jīng)病變血瘀脈阻型患者雙下肢脛神經(jīng)傳導(dǎo)速度水平。
[Abstract]:Objective: to study the treatment of diabetic patients with lower extremity neuropathy by injecting vitamin B1 and vitamin B6 at Zusanli acupoint, and observe the improvement of nerve conduction velocity, symptoms and signs of lower extremities before and after treatment. To confirm the therapeutic effect of Zusanli acupoint injection on diabetic peripheral neuropathy of lower extremity, and to provide a reliable method for the treatment of diabetic lower extremity neuropathy. Methods: 120 patients with diabetic lower extremity neuropathy who met the inclusion criteria were selected. The patients were randomly divided into four groups: group A (30 cases), group B (30 cases) basic treatment, group B ( All the patients in the four groups were treated for 2 weeks. Observe TCM syndromes integral, change of tibial nerve conduction velocity (NCV) of lower extremity, collect data, and get the result by statistical analysis. The result is 1: 1. In the comparison of TCM syndromes score, the comparison between group D and the other three groups showed that the difference was statistically significant (P0.05) between), B group and Agna C group (P0.05); intra-group comparison showed that group B and group D were compared before and after treatment, the difference between group D and group D was significant before and after treatment (P0.05), and the difference between group D and group D was significant before and after treatment (P0.05). All had statistical significance (P0.05). Compared with other groups, the total effective rate of B6 group was 86.67, compared with other groups, P0.05, all had statistical significance. The comparison of nerve conduction velocity after treatment showed that the difference between group D and other three groups was statistically significant (P0.05), B group and C group were compared with group A, respectively). There was no significant difference between group D and group C (P0.05), B and group C, P0.05). Group B, group C and group D were compared before and after treatment, with statistical significance (P0.05). Conclusion 1. Injection of vitamin B1 and B6 at Zusanli acupoint can significantly improve the symptoms and signs of patients with diabetic lower extremity neuropathy with blood stasis and pulse obstruction. 2. Injection of vitamin B1 and B6 at Zusanli acupoint could significantly improve the tibial nerve conduction velocity in patients with diabetic lower extremity neuropathy with blood stasis and pulse obstruction.
