【摘要】：目的:目前國(guó)內(nèi)、外有關(guān)血尿酸(UA)對(duì)2型糖尿病大血管病影響的研究比較普遍,而有關(guān)血小板指數(shù)與血尿酸(UA)的相關(guān)性研究比較少,本研究通過(guò)分析2型糖尿病患者頸動(dòng)脈病變與血尿酸(UA)及血小板指數(shù)的相關(guān)性,為2型糖尿病患者頸動(dòng)脈粥樣硬化的早期防治提供理論依據(jù)。同時(shí)也為我國(guó)許多不具備超聲或核磁共振等的大型設(shè)備檢查的基層醫(yī)院及鄉(xiāng)鎮(zhèn)衛(wèi)生院,初步推斷早期2型糖尿病動(dòng)脈硬化提供一定的理論參考。方法:近2年大連大學(xué)附屬中山醫(yī)院內(nèi)分泌科住院治療的2型糖尿病患者為研究對(duì)象,定義為研究組,檢測(cè)頸動(dòng)脈超聲測(cè)定內(nèi)-中膜厚度,根據(jù)頸動(dòng)脈內(nèi)-中膜厚度分為2亞組:頸動(dòng)脈增厚組138例(1.0mm內(nèi)-中膜厚度≤1.2mm)。頸動(dòng)脈增厚并斑塊組154例(頸動(dòng)脈內(nèi)中膜厚度1.2mm)。以同期來(lái)我院進(jìn)行健康檢查的健康人群為對(duì)照組,共236例。比較研究組及對(duì)照組各血液指標(biāo)的差異。結(jié)果:(1)研究組及對(duì)照組總膽固醇(TC)、血清甘油三酯(TG)、血清低密度脂蛋白膽固醇(LDL-C)、血清高密度脂蛋白膽固醇(HDL-C)、體重指數(shù)(BMI)、血小板壓積(PCT)、血小板分布寬度(PDW)、血小板計(jì)數(shù)(PLT)差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。(2)研究組糖化血紅蛋白(HBA1C)、空腹血糖(FPG)、血尿酸(UA)、平均血小板體積(MPV)均高于對(duì)照組(P0.05)(3)頸動(dòng)脈增厚組中尿酸增高者M(jìn)PV高于非尿酸增高者(P0.05),頸動(dòng)脈增厚組中尿酸增高者與非尿酸增高者,PLT、PCT、PDW差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。(4)頸動(dòng)脈增厚并斑塊組中尿酸增高者M(jìn)PV高于非尿酸增高者(P0.05),頸動(dòng)脈增厚組中尿酸增高者與非尿酸增高者,PLT、PCT、PDW差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:2型糖尿病患者出現(xiàn)頸動(dòng)脈病變后,血尿酸水平升高。血尿酸可作為預(yù)測(cè)2型糖尿病外周血管病變的指標(biāo),MPV與血尿酸增高也具有一定的相關(guān)性。
[Abstract]:Objective: at present, the effect of serum uric acid (UA) on type 2 diabetic macroangiopathy is common in China and abroad, but the correlation between platelet index and serum uric acid (UA) is less. In this study, the relationship between carotid artery lesions and serum uric acid (UA) and platelet index in type 2 diabetes mellitus was analyzed to provide theoretical basis for early prevention and treatment of carotid atherosclerosis in patients with type 2 diabetes mellitus. At the same time, it also provides a theoretical reference for many basic hospitals and township hospitals that do not have ultrasound or nuclear magnetic resonance examination, and preliminarily infer early type 2 diabetic arteriosclerosis. Methods: type 2 diabetic patients who were hospitalized in Department of Endocrinology of Zhongshan Hospital affiliated to Dalian University in recent 2 years were studied and defined as study group.
