【摘要】：隨著糖尿病和骨質(zhì)疏松癥在我國的廣泛流行,糖尿病性骨質(zhì)疏松癥已成為糖尿病患者致死、致殘的重要原因,嚴(yán)重影響患者的生活質(zhì)量,并給個(gè)人、社會帶來沉重負(fù)擔(dān)。1型糖尿病患者骨密度降低,骨折風(fēng)險(xiǎn)增加;2型糖尿病患者骨密度常增高或正常,但骨折風(fēng)險(xiǎn)也是增加的,這不能僅靠雙能X線骨密度來解釋。骨轉(zhuǎn)換標(biāo)志物具有靈敏度高、特異性強(qiáng)、穩(wěn)定性好等優(yōu)點(diǎn),近年來在糖尿病中得到廣泛研究,如骨堿性磷酸酶、1型原膠原N-端前肽、1型膠原交聯(lián)C-末端肽、骨鈣素、骨保護(hù)素、脫氧吡啶啉等。骨轉(zhuǎn)換標(biāo)志物反映骨吸收和骨形成的具體變化情況,反映骨強(qiáng)度,較骨密度更早的反映骨量變化,大量臨床研究發(fā)現(xiàn),它為臨床早期發(fā)現(xiàn)和診斷糖尿病性骨質(zhì)疏松癥,評估糖尿病患者骨折風(fēng)險(xiǎn)提供了新思路。聯(lián)合檢測骨轉(zhuǎn)換標(biāo)志物和骨密度,更全面、合理的評估骨轉(zhuǎn)換,及時(shí)發(fā)現(xiàn)高危人群,更有利于糖尿病性骨質(zhì)疏松癥患者的早期診斷及治療,預(yù)防骨折的發(fā)生。本文將對骨轉(zhuǎn)換標(biāo)志物在糖尿病中的研究進(jìn)展作一綜述。
[Abstract]:With the prevalence of diabetes and osteoporosis in our country, diabetic osteoporosis has become an important cause of death and disability in patients with diabetes, seriously affecting the quality of life of patients and giving it to individuals. Society brings a heavy burden. Type 1 diabetes patients have lower bone density and higher fracture risk. Type 2 diabetes patients often have higher or normal bone mineral density, but the fracture risk is also increased, which can not be explained by dual energy X-ray bone mineral density alone. Bone conversion markers have been widely studied in recent years, such as bone alkaline phosphatase, type 1 procollagen N-terminal prepeptide, type 1 collagen cross-linked C-terminal peptide, osteocalcin, osteoprotegerin, and so on. Deoxypyridine, etc. Bone turnover markers reflect specific changes of bone resorption and bone formation, reflect bone strength, and reflect bone mass changes earlier than bone mineral density. A large number of clinical studies have found that it is an early clinical diagnosis of diabetic osteoporosis. The evaluation of fracture risk in diabetic patients provides a new idea. Combined detection of bone turnover markers and bone mineral density, more comprehensive and reasonable evaluation of bone turnover, timely detection of high-risk population, is more conducive to the early diagnosis and treatment of patients with diabetic osteoporosis, and to prevent the occurrence of fractures. This article reviews the progress of bone turnover markers in diabetes mellitus.
