【摘要】：背景:目前骨質(zhì)疏松性椎體壓縮性骨折患者常見,但缺乏規(guī)范抗骨質(zhì)疏松治療,使得治療后再骨折問題逐漸顯現(xiàn)。目的:探討椎體成形技術(shù)配合抗骨質(zhì)疏松治療骨質(zhì)疏松性椎體壓縮性骨折后減少再骨折發(fā)生率方面的療效,分析抗骨質(zhì)疏松治療的重要性。方法:納入89例骨質(zhì)疏松性椎體壓縮性骨折患者,所有患者均行椎體成形/椎體后凸成形治療,根據(jù)患者是否接受規(guī)范的抗骨質(zhì)疏松治療分為治療組38例、對照組51例,通過門診或住院隨訪復查胸/腰椎X射線片及骨密度值,對比兩組在隨訪不同時間的脊柱穩(wěn)定性、骨密度值及椎體再骨折發(fā)生率。結(jié)果與結(jié)論:(1)78例獲得完整隨訪,隨訪6-39個月,平均26.73個月,治療組、對照組在脊柱穩(wěn)定性方面差異無顯著性意義(P0.05),但對照組椎體骨小梁稀少;(2)骨密度值:兩組治療后12,24,36個月T值差異有顯著性意義(P0.05);(3)再骨折發(fā)生率:治療組明顯低于對照組,差異有顯著性意義(P0.05);(4)結(jié)果表明,骨質(zhì)疏松性椎體壓縮性骨折椎體成形治療后規(guī)范的抗骨質(zhì)疏松治療可有效減少再骨折發(fā)生率,近、中期療效滿意。
[Abstract]:Background: at present, osteoporosis vertebral compression fracture patients are common, but lack of standardized anti-osteoporosis treatment, make the problem of re-fracture gradually appear after treatment. Objective: to investigate the effect of vertebroplasty combined with anti-osteoporosis in reducing the incidence of refracture after osteoporotic vertebral compression fracture, and to analyze the importance of anti-osteoporosis treatment. Methods: 89 patients with osteoporotic vertebral compression fracture were treated with vertebroplasty / kyphoplasty. According to whether the patients received regular anti-osteoporosis treatment, they were divided into treatment group (n = 38) and control group (n = 51). The spine stability, bone mineral density and the incidence of vertebral refracture were compared between the two groups after out-patient or in-patient follow-up. Results and conclusion: (1) 78 cases were followed up for 6-39 months (mean 26.73 months). There was no significant difference in spinal stability in the control group (P0.05), but the bone trabeculae in the control group were scarce; (2) bone mineral density: after treatment, there was significant difference in T value between the two groups (P0.05); (3): the incidence of refracture in the treatment group was significantly lower than that in the control group. The difference was significant (P0.05); (4). The results showed that the standard anti-osteoporosis treatment after vertebroplasty treatment for osteoporotic vertebral compression fracture could effectively reduce the incidence of refracture and the curative effect was satisfactory in the near and medium term.
