ANCA相關(guān)性血管炎伴血清補體C3降低的臨床分析

【摘要】：目的探討血清補體C3(serum complement C3,sC3)降低的抗中性粒細胞胞漿抗體相關(guān)性血管炎(AAV)患者的臨床病理特征及預(yù)后。方法回顧性分析2011年1月至2016年3月我院腎內(nèi)科穿刺活檢確診的54例AAV患者的臨床及病理資料。采用免疫比濁法檢測sC3,以我院sC3測值的正常范圍下線值為截點將AAV患者分成低sC3組(sC30.9 g/L)與正常sC3組(sC3≥0.9 g/L),比較2組患者流行病學(xué)、實驗室檢查、腎臟病理、臨床癥狀、治療及預(yù)后的差異。結(jié)果低sC3組,血肌酐基線值明顯高于正常sC3組(P=0.008);C反應(yīng)蛋白、eGFR值明顯低于正常sC3組(P值分別0.001、0.012);快速腎功能衰竭以及即時透析的發(fā)生率明顯高于正常sC3組(P值分別為0.035、0.014);緩解率顯著低于正常sC3組(P0.001)。Kaplan-Meier生存分析顯示低sC3組的腎臟累積生存率明顯低于正常sC3組(log-rank=10.197,P=0.001)。結(jié)論血清補體C3降低的AAV患者病情嚴(yán)重,進展較快,預(yù)后差。
