HbA1c水平對2型糖尿病患者紅細胞及胞內(nèi)血紅蛋白的影響

【摘要】:目的:探討不同糖化血紅蛋白(Hb A1c)水平對2型糖尿病(T2DM)患者紅細胞及胞內(nèi)血紅蛋白(Hb)的影響。方法:選取健康人30名為對照組(H組),T2DM患者102例,按Hb A1c水平分為3組:血糖控制良好組(A組)30例(Hb A1c7.0%);血糖控制較差組(B組)36例(7.0%≤Hb A1c9.0%);持續(xù)高血糖組(C組)36例(Hb A1c≥9.0%)。利用新型靜態(tài)顯微圖像分析技術(shù)和紫外-可見光譜技術(shù),對各組紅細胞及胞內(nèi)Hb進行檢測。結(jié)果:與H組比較,A、B、C組紅細胞的圓度因子明顯增大(P0.05)且與Hb A1c水平呈正相關(guān);B組紅細胞的截面積和C組紅細胞的全部形態(tài)參數(shù)均較H組明顯增大(P0.05);A、B、C組,除A、B組間紅細胞長短軸參數(shù)無顯著差異,其它組間參數(shù)比較差異有統(tǒng)計學顯著性。與H組比較,A、B、C組Hb的紫外-可見光譜的譜型和譜峰未出現(xiàn)明顯差異,但B、C組Hb的吸光度較H組明顯下降(P0.01),且C組Hb的吸光度也明顯低于A組(P0.05),隨Hb A1c水平的升高呈逐漸下降趨勢。結(jié)論:隨Hb A1c水平的升高,T2DM患者紅細胞的形態(tài)發(fā)生改變,變形功能逐漸減弱,胞內(nèi)Hb的結(jié)構(gòu)可能發(fā)生改變。
[Abstract]:Objective: To investigate the effect of different levels of glycosylated hemoglobin (Hb A1c) on erythrocyte and intracellular hemoglobin (Hb) in patients with type 2 diabetes mellitus (T2DM). Results: Compared with group H, the roundness factor of erythrocyte in group A, B and C was significantly increased (P 0.05) and positively correlated with the level of Hb A1c. All the morphological parameters of red blood cells in group C were significantly higher than those in group H (P 0.05); there was no significant difference in the long and short axis parameters of red blood cells between groups A, B and C except group A and B, and there was significant difference in other parameters between groups. The absorbance of Hb in group C was also significantly lower than that in group A (P 0.05). With the increase of Hb A1c level, the red blood cell morphology, deformation function and intracellular Hb structure in patients with T2DM changed gradually.
