【摘要】：目的:探討蒼附導(dǎo)痰湯聯(lián)合枸櫞酸氯米芬膠囊對多囊卵巢綜合征(PCOS)合并不孕癥患者血清性激素水平及子宮內(nèi)膜容受性的影響。方法:選取2014年1月-2014年12月在貴陽中醫(yī)學(xué)院第一附屬醫(yī)院確診為PCOS合并不孕癥的患者116例,按照隨機數(shù)字表法分為觀察組和對照組,各58例。兩組患者均于月經(jīng)第5天起給予調(diào)節(jié)激素水平治療,對照組同時給予枸櫞酸氯米芬膠囊50 mg,qd;觀察組患者在對照組基礎(chǔ)上加服蒼附導(dǎo)痰湯1劑,水煎取汁400 mL,bid,兩組患者均治療3個月經(jīng)周期。比較兩組患者治療前后血清性激素水平[雌二醇(E_2)、黃體生成素(LH)]、子宮內(nèi)膜厚度(Em)、子宮內(nèi)膜螺旋動脈搏動指數(shù)(PI)、阻力指數(shù)(RI),隨訪1年比較兩組患者排卵率及妊娠率。結(jié)果:治療前,兩組患者E_2、LH、Em、PI、RI比較,差異均無統(tǒng)計學(xué)意義(P0.05);治療后,兩組患者E_2、LH、Em水平顯著升高,PI、RI顯著降低,且觀察組顯著優(yōu)于對照組,差異均有統(tǒng)計學(xué)意義(P0.05)。隨訪1年,兩組患者排卵率(77.59%vs.72.41%)比較,差異無統(tǒng)計學(xué)意義(P0.05);觀察組患者妊娠率為41.38%,顯著高于對照組的22.41%,差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:蒼附導(dǎo)痰湯聯(lián)合枸櫞酸氯米芬膠囊能有效調(diào)節(jié)PCOS合并不孕癥患者的血清性激素水平,改善子宮內(nèi)膜容受性,從而提高妊娠率。
[Abstract]:Objective: to investigate the effect of Cangfu Doduantan decoction combined with clomiphene citrate capsule on serum sex hormone level and endometrial receptivity in patients with polycystic ovary syndrome (PCOS) complicated with infertility. Methods: from January 2014 to December 2014, 116 patients with PCOS combined with infertility were selected and divided into observation group (n = 58) and control group (n = 58) in the first affiliated Hospital of Guiyang College of traditional Chinese Medicine from January 2014 to December 2014. The patients in both groups were treated with regulating hormone level from the 5th day of menstruation, the control group was treated with clomiphene citrate capsule 50 mg / g QD, and the patients in the observation group were treated with Cangfu Duan phlegm decoction 1 dose on the basis of the control group. The patients in both groups were treated for 3 menstrual cycles. The serum sex hormone levels (estradiol 2, luteinizing hormone (LH), endometrial thickness (Em), endometrial spiral artery pulsatility index (PI), resistance index) and (PI), resistance index (RI),) were compared before and after treatment between the two groups for one year, and the ovulation rate and pregnancy rate were compared between the two groups. Results: before treatment, there was no significant difference between the two groups (P 0.05), but after treatment, the level of E2LHHN Em in the two groups was significantly higher than that in the control group (P0.05), and the observation group was significantly better than the control group (P0.05). The rate of ovulation in the two groups (77.59v s.72.41%) was not significantly different (P0.05), and the pregnancy rate in the observation group was 41.38, which was significantly higher than that in the control group (22.41) (P0.05). Conclusion: Cangfu Doutan decoction combined with clomiphene citrate capsule can effectively regulate the level of serum sex hormone in PCOS patients with infertility, improve endometrial receptivity and increase pregnancy rate.
