2型糖尿病患者血清鋅離子水平與氧化應(yīng)激及內(nèi)皮損傷的研究

【摘要】:背景: 糖尿病(diabetes mellitus,DM)是由遺傳和環(huán)境因子共同作用而引起的以胰島素及β細(xì)胞功能障礙致糖代謝紊亂為主要表現(xiàn)的臨床綜合征。近年研究顯示,糖尿病與氧化應(yīng)激(oxidative stress,OS)有密切聯(lián)系。氧化應(yīng)激是指機體在遭受各種有害刺激,生成體內(nèi)高活性分子,如活性氧自由基和活性氮自由基的產(chǎn)生過多,氧化程度超出氧化物的清除,氧化系統(tǒng)和抗氧化系統(tǒng)失衡,導(dǎo)致組織損傷。丙二醛(malondialdehyde,MDA)是氧化應(yīng)激的指標(biāo),而超氧化物岐化酶superoxide dismutase,SOD)則是抗氧化系統(tǒng)的指標(biāo)。研究認(rèn)為內(nèi)皮細(xì)胞功能紊亂是糖尿病心血管并發(fā)癥的始動因素。血管內(nèi)皮細(xì)胞分泌的多種活性因子一氧化氮(nitric oxide,NO)和內(nèi)皮素(endothelin,ET)可導(dǎo)致內(nèi)皮功能障礙。鋅離子參與胰島素的合成、分泌、生物活性和抗原性,鋅與糖尿病的發(fā)生、發(fā)展及并發(fā)癥的發(fā)生都密切相關(guān)。同時,鋅是維持細(xì)胞正常功能的必需金屬元素,鋅可作為多種酶和蛋白質(zhì)的輔助因子參與抗氧化作用。因此,本研究通過探討鋅離子對2型糖尿病患者氧化應(yīng)激的影響及對2型糖尿病患者血清內(nèi)皮功能的影響,尋找干預(yù)靶點進(jìn)行有效的預(yù)防及治療。 目的: 本實驗通過檢測對照組及糖尿病組血清氧化應(yīng)激指標(biāo)(SOD、MDA)及內(nèi)皮損傷因子(NO、ET)的含量水平,探討鋅離子與氧化應(yīng)激及內(nèi)皮損傷因子的關(guān)系,明確鋅對糖尿病患者心血管并發(fā)癥的保護作用。 方法: 吉大一院老年病科就診2型糖尿病患者為糖尿病組,同時來自醫(yī)院就診的非糖尿病患者為對照組,研究對象的基本資料、調(diào)查患者情況、近期用藥情況。檢測所有研究對象血清中鋅離子水平。采用試劑盒測定研究對象血清中NO、ET及SOD、MDA。分析研究對象血清中鋅離子水平,,研究對象血清中NO、ET、MDA、SOD與鋅離子水平的相關(guān)性。 結(jié)果: 1、糖尿病組患者血清鋅離子水平明顯低于對照組血清鋅離子水平,有顯著統(tǒng)計學(xué)差異(P0.05)。 2、糖尿病組與對照組相比患者抗氧化指標(biāo)SOD明顯下降,氧化應(yīng)激指標(biāo)MDA明顯升高,有顯著統(tǒng)計學(xué)差異(P0.05)。 3、糖尿病組患者與對照組相比內(nèi)皮損傷因子NO、ET水平升高,有統(tǒng)計學(xué)意義(P0.05)。 結(jié)論: 1、2型糖尿病患者血清離子發(fā)生紊亂,其中鋅離子、鎂離子在糖尿病組患者血清中明顯下降。 2、2型糖尿病患者抗氧化能力明顯下降,同時氧化損傷明顯增加。 3、2型糖尿病組患者內(nèi)皮功能明顯受損。
[Abstract]:Background: diabetes mellitus (diabetes mellitus DM) is a clinical syndrome caused by the interaction of genetic and environmental factors, which is mainly characterized by impaired glucose metabolism caused by insulin and 尾 cell dysfunction. Recent studies have shown that diabetes is closely associated with oxidative stress. Oxidative stress refers to the excessive production of highly active molecules such as reactive oxygen free radicals and active nitrogen free radicals in the body under various harmful stimuli, the degree of oxidation exceeding the removal of oxides, and the imbalance between the oxidation system and the antioxidant system. Causing tissue damage. Malondialdehyde (malondialdehyde) is an index of oxidative stress, while superoxide dismutase (superoxide) is an index of antioxidant system. It is believed that endothelial cell dysfunction is the initiator of cardiovascular complications in diabetes mellitus. Endothelial dysfunction can be caused by nitric oxide (no) and endothelin et (et) secreted by vascular endothelial cells. Zinc ion is involved in the synthesis, secretion, bioactivity and antigenicity of insulin. Zinc is closely related to the occurrence, development and complications of diabetes mellitus. At the same time, zinc is an essential metal element to maintain the normal function of cells. Zinc can be used as an auxiliary factor of many enzymes and proteins to participate in antioxidation. Therefore, in this study, the effect of zinc ion on oxidative stress and serum endothelial function in type 2 diabetes mellitus patients was investigated to find effective intervention targets for prevention and treatment. Objective: to investigate the relationship between zinc ion and oxidative stress and endothelial injury factor by detecting the levels of serum oxidative stress index (SOD- MDA) and endothelial injury factor (NO-et) in control group and diabetic group. To determine the protective effect of zinc on cardiovascular complications in diabetic patients. Methods: type 2 diabetic patients in the geriatric department of Jida first Hospital were treated as diabetic group and non-diabetic patients in hospital as control group. The basic data of the subjects were investigated and the recent drug use was investigated. Serum zinc ion levels were measured in all subjects. The determination of NONET and SODX MDAs in serum was carried out by using the kit. The level of zinc ion in serum and the correlation between SOD and zinc ion level in serum were analyzed. Results: 1. The serum zinc ion level in the diabetic group was significantly lower than that in the control group (P0.05). Oxidative stress index MDA significantly increased, there was significant difference (P0.05). 3, compared with the control group, the level of endothelium injury factor NON-ET in diabetic patients increased significantly (P0.05). Conclusion: (1) the serum ions of type 2 diabetes mellitus patients have disorder, among which zinc ion and magnesium ion are obviously decreased in the serum of the patients with type 2 diabetes mellitus, and the antioxidant ability of type 2 diabetes mellitus patients is obviously decreased. At the same time, oxidative injury was significantly increased. Endothelial function was significantly impaired in type 3 diabetes mellitus patients.
