Betatrophin與胰島素抵抗和胰島β細(xì)胞功能的關(guān)系研究

【摘要】:目的:通過檢測血糖正常者、2型糖尿病患者血清betatrophin水平、血糖、胰島素、血脂等指標(biāo),分析血清betatrophin水平與胰島素抵抗和胰島β細(xì)胞功能的關(guān)系及與血脂等其他指標(biāo)的相關(guān)性,探討betatrophin對人體糖脂代謝的臨床價(jià)值,為其今后的臨床應(yīng)用提供線索和依據(jù)。方法:選取2015年09月-2016年4月在蚌埠醫(yī)學(xué)院第一附屬醫(yī)院內(nèi)分泌科實(shí)驗(yàn)室檢查糖耐量正常者78位,根據(jù)空腹胰島素(FINS)水平分兩組,FINS12.0m IU/L指定為對照組(NC組n=49),FINS≥12.0mIU/L為高胰島素血癥組(HINS組n=29),選取內(nèi)分泌科住院治療已確診的2型糖尿病(T2DM)患者33例為T2DM組(n=33)。對各組進(jìn)行問卷、體格檢查、檢測受試者基本臨床指標(biāo),并采用酶聯(lián)免疫吸附法測定betatrophin水平,放射免疫法測定25羥維生素D(25-OH-VD)水平。運(yùn)用SPSS17.0軟件分析比較各組指標(biāo)的差異性及血清betatrophin水平與各指標(biāo)的相關(guān)性。結(jié)果:1、T2DM組血清betatrophin水平明顯高于非糖尿病組(NDM組);HINS組血清betatrophin水平明顯低于NC組。T2DM組血清25-OH-VD水平明顯低于NDM組。2、所有指標(biāo)與betatrophin濃度的相關(guān)性。血清betatrophin水平與年齡、腰臀比、總膽固醇(TCH)、低密度脂蛋白(LDL)呈正相關(guān);與體重指數(shù)(BMI)、臀圍、空腹胰島素(FINS)、穩(wěn)態(tài)模型胰島素抵抗指數(shù)(HOMA-IR)、穩(wěn)態(tài)模型胰島β細(xì)胞功能指數(shù)(HOMA-β)、25-OH-VD呈負(fù)相關(guān);血清betatrophin水平與性別、體重、腰圍、甘油三酯(TG)、高密度脂蛋白(HDL)、FPG無相關(guān)性。在NDM組中,血清betatrophin水平與年齡呈正相關(guān),與BMI、FINS、HOMA-IR、HOMA-β呈負(fù)相關(guān)。多元線性逐步回歸分析:年齡和血清25-OH-VD水平是betatrophin的獨(dú)立影響因素。在NDM組中,血清betatrophin水平與年齡相關(guān)。3、組間臨床指標(biāo)差異比較,NC組、HINS組和T2DM組之間性別、身高、舒張壓、天冬氨酸氨基轉(zhuǎn)氨酶(AST)、HDL無明顯差異。年齡和FPG明顯偏高在T2DM組。在血脂方面,T2DM組TCH、LDL水平明顯偏高。HINS組中體重、BMI、臀圍、FINS水平明顯高于NC組和T2DM組,三組間HOMA-IR、HOMA-β有顯著差異,HOMA-IR在HINS組、T2DM組、NC組呈遞減,HOMA-β在HINS組、NC組、T2DM組逐漸降低。4、組內(nèi)相關(guān)分析顯示,在T2DM組未發(fā)現(xiàn)血清betatrophin水平與任何指標(biāo)有相關(guān)性。HINS組血清betatrophin水平與年齡、腰圍有相關(guān)性,NC組betatrophin水平與年齡、AST、TG、FINS相關(guān)。5、血清25-OH-VD水平與HOMA-IR正相關(guān),與年齡、betatrophin呈負(fù)相關(guān),與其他檢測指標(biāo)無相關(guān)性。結(jié)論:1.在高胰島素血癥狀態(tài)下,血清betatrophin水平降低;胰島β細(xì)胞功能降低的T2DM患者血清betatrophin水平升高,提示血清betatrophin水平與胰島素抵抗、胰島β細(xì)胞功能相關(guān),betatrophin可能參與糖尿病的發(fā)生發(fā)展。2.年齡和血清25-OH-VD水平是血清betatrophin水平的獨(dú)立影響因素。
