不同糖耐量人群血漿血管內(nèi)皮生長(zhǎng)因子-B水平與胰島素抵抗的相關(guān)性研究

【摘要】:目的:檢測(cè)不同糖耐量人群血漿血管內(nèi)皮生長(zhǎng)因子B(VEGF-B)水平變化,探討其與糖脂代謝、肥胖及胰島素抵抗(IR)的關(guān)系。方法:納入新診斷2型糖尿病患者(T2DM組)、糖耐量異常者(IGT組)、糖耐量正常者(NGT)組共261例。行葡萄糖耐量試驗(yàn),并同時(shí)檢測(cè)血糖、胰島素、糖化血紅蛋白(Hb A1C)、血脂等指標(biāo),計(jì)算HOMA-β、HOMA-IR評(píng)估胰島功能及胰島素抵抗;再以BMI=25kg/m2為切點(diǎn)將其分為非肥胖組(包括正常體重者和超重者)和肥胖組。ELISA測(cè)定不同糖耐量者血漿VEGF-B水平,分析血漿VEGF-B水平與相關(guān)代謝指標(biāo)及胰島素抵抗的關(guān)系。結(jié)果:T2DM組、IGT組血漿VEGF-B水平高于NGT組[(194.87±61.80)vs(148.46±45.68)vs(111.64±32.49)pg/ml,P0.01]??cè)巳褐蟹逝纸M血漿VEGF-B水平高于非肥胖組[(169.39±60.58)vs(118.91±48.88)pg/ml,P0.01]。相關(guān)性分析顯示,血漿VEGF-B水平與BMI、WHR、TG、FFA、FPG、FIns、HOMA-IR呈正相關(guān)(P0.01),進(jìn)一步矯正BMI、WHR后,血漿VEGF-B水平與HOMA-IR仍呈正相關(guān)。多元線性逐步回歸分析表明,T2DM組HOMA-IR、BMI是血漿VEGF-B水平的獨(dú)立影響因素(R2=0.405、0.432,P0.05)。結(jié)論:血漿VEGF-B水平在新診斷T2DM、IGT及肥胖人群中升高,且其與糖脂代謝、IR有關(guān),可能參與2型糖尿病的發(fā)生與發(fā)展。
[Abstract]:Aim: to detect the changes of plasma vascular endothelial growth factor (VEGF-B) levels in different glucose tolerance subjects and to explore the relationship between VEGF-B and glucose and lipid metabolism, obesity and insulin resistance (IRR). Methods: a total of 261 patients with newly diagnosed type 2 diabetes mellitus (T2DM), IGT with impaired glucose tolerance (IGT) and NGT with normal glucose tolerance (NGT) were enrolled. Glucose tolerance test was performed, blood glucose, insulin, glycosylated hemoglobin HbA1CX, blood lipids were measured at the same time. HOMA- 尾 -HOMA-IR was used to evaluate islet function and insulin resistance. The plasma VEGF-B levels were determined by Elisa in non-obese group (including normal weight group and overweight group) and obese group. The relationship between plasma VEGF-B level and related metabolic indexes and insulin resistance was analyzed. Results the level of VEGF-B in the IGT group was higher than that in the NGT group [194.87 鹵45.68)vs(111.64 鹵32.49 PG / ml P0.01]. The plasma VEGF-B level in obese group was higher than that in non-obese group (169.39 鹵48.88 60.58)vs(118.91 鹵48.88 PG / ml P0.01). Correlation analysis showed that plasma VEGF-B level was positively correlated with HOMA-IR of BMI-WHRG / FFAFP FInsInsn, and the plasma VEGF-B level was still positively correlated with HOMA-IR after further correction of BMI-WHR. Multiple linear stepwise regression analysis showed that HOMA-IRN BMI was an independent factor influencing plasma VEGF-B level in T2DM group. Conclusion: the plasma VEGF-B level is increased in newly diagnosed T2DMN IGT and obese population, and it may be involved in the occurrence and development of type 2 diabetes mellitus.
