【摘要】：目的探討腓腸神經(jīng)超聲檢查在糖尿病周圍神經(jīng)病變(DPN)患者中的應(yīng)用價(jià)值。方法 82例DPN患者(DPN組)、82例2型糖尿病(T2DM)非DPN患者(非DPN組)及82例健康志愿者(對(duì)照組)進(jìn)行腓腸神經(jīng)超聲學(xué)檢查,觀察腓腸神經(jīng)形態(tài)學(xué)改變,并采用肌電圖診斷儀測(cè)量腓腸神經(jīng)傳導(dǎo)速度。結(jié)果糖尿病(DM)患者腓腸神經(jīng)超聲下多表現(xiàn)神經(jīng)外膜模糊、篩網(wǎng)狀結(jié)構(gòu)不清、神經(jīng)束回聲減低、與鄰近組織分界不清,DPN組上述超聲表現(xiàn)比例均高于非DPN組(P0.05)。DM患者超聲下腓腸神經(jīng)橫徑、厚徑及截面積均高于對(duì)照組,腓腸神經(jīng)傳導(dǎo)速度低于對(duì)照組;DPN組超聲下腓腸神經(jīng)橫徑、厚徑及截面積均高于非DPN組,腓腸神經(jīng)傳導(dǎo)速度低于非DPN組(P0.05)。結(jié)論高頻超聲能清晰顯示DDPN患者腓腸神經(jīng)形態(tài)學(xué)改變,表現(xiàn)為腓腸神經(jīng)增粗、腫脹。DM患者在發(fā)展成DPN之前周圍神經(jīng)已經(jīng)出現(xiàn)了異常。
[Abstract]:Objective to evaluate the value of sural nerve ultrasonography in patients with diabetic peripheral neuropathy (DPN). Methods 82 cases of DPN patients with DPN and 82 cases of non-DPN patients (non-DPN group) and 82 healthy volunteers (control group) were examined by sural nerve ultrasound, and the morphological changes of sural nerve were observed. The conduction velocity of sural nerve was measured by electromyography. Results the sural nerve in patients with DM (DM) showed that the epineurium was blurred, the ethmoid structure was not clear, and the echo of nerve bundles was decreased. In DPN group, the transverse diameter, thickness and area of sural nerve in DPN group were higher than those in non-DPN group, and the conduction velocity of sural nerve in DPN group was lower than that in DPN group, and the transverse diameter of sural nerve in DPN group was lower than that in DPN group, and the diameter of sural nerve in DPN group was lower than that in DPN group. Thick diameter and cross section area were higher than that of non DPN group, and the conduction velocity of sural nerve was lower than that of non DPN group (P 0.05). Conclusion the morphological changes of sural nerve in DDPN patients can be clearly demonstrated by high frequency ultrasound. The sural nerve is thickened and swollen. The peripheral nerve has been abnormal before developing into DPN.
