動態(tài)血糖監(jiān)測系統(tǒng)監(jiān)測與其他葡萄糖檢測方法的臨床比較

【摘要】:目的探討動態(tài)血糖監(jiān)測(CGF)和靜脈血漿葡萄糖、毛細(xì)血管全血葡萄糖的關(guān)系。以血糖的"穩(wěn)態(tài)"為基礎(chǔ),側(cè)重于分析非"穩(wěn)態(tài)"狀態(tài)下,靜脈血漿葡萄糖測定和CGF數(shù)據(jù)的差異和相關(guān)性。方法取自2015年1~8月云南省第一人民醫(yī)院內(nèi)分泌代謝科住院和門診患者32例,其中2型糖尿病29例,低血糖查因3例。采用葡糖氧化酶(GOD)CGF和動態(tài)血糖監(jiān)測系統(tǒng)(DGMS)3種方法測定血糖,比較各組數(shù)據(jù)間的差異和相關(guān)性。結(jié)果 DGMS(122)數(shù)值略低于CGF(122)測定值,兩者雖然僅相差(0.11±0.35)mmol/L,但差異具有統(tǒng)計學(xué)意義(P=0.001)。口服葡萄糖耐量試驗(yàn)(OGTT)過程中,0 min GOD低于DGMS(P=0.000),30 min起GOD迅速超過DGMS(P=0.001),從120 min開始DGMS上升的速度快于GOD,雖然仍較GOD為低,但兩者間差異已沒有統(tǒng)計學(xué)意義(P0.05)。合并5組數(shù)據(jù)后,GOD(160)與DGMS(160),均數(shù)比較,差異仍有統(tǒng)計學(xué)意義(P=0.000),提示血漿葡萄糖的濃度整體水平高于組織液。CGF(122)與DGMS(122)相關(guān)系數(shù)為0.989(P=0.000),GOD(160)與DGMS(160)相關(guān)系數(shù)為0.911(P=0.000)。結(jié)論在穩(wěn)態(tài)狀態(tài)下,CGF全血葡萄糖與DGMS之間有非常好的相關(guān)關(guān)系,并以穩(wěn)態(tài)狀態(tài)為基礎(chǔ),比較OGTT或饅頭餐耐量試驗(yàn)過程中,隨著血糖快速升高,DGMS監(jiān)測的葡萄糖濃度也隨之升高,變化趨勢與GOD相類似,但是絕對值顯示DGMS的反應(yīng)較GOD慢,進(jìn)餐后到120 min以前DGMS測定值偏低,但120 min起DGMS與GOD血糖濃度差異已經(jīng)沒有統(tǒng)計學(xué)意義,這是在非穩(wěn)態(tài)狀態(tài)下做出的結(jié)果,反應(yīng)了真實(shí)的血糖情況,而且綜合5個點(diǎn)的分析,最終的結(jié)果是DGMS和GOD有很好的相關(guān)性。
[Abstract]:Objective to investigate the relationship between CGF and venous plasma glucose and capillary whole blood glucose. Based on the "steady-state" of blood glucose, the difference and correlation between venous plasma glucose measurement and CGF data were analyzed. Methods from January to August 2015, 32 inpatients and outpatients from the Department of Endocrinology and Metabolism of the first people's Hospital of Yunnan Province were collected, including 29 cases of type 2 diabetes and 3 cases of hypoglycemia. Three methods were used to measure blood glucose by GODGF and DGMS. The difference and correlation between the data of each group were compared. Results the value of DGMS ~ (122) was slightly lower than that of CGF ~ (122). Although the difference was only 0.11 鹵0.35 渭 mol / L, the difference was statistically significant (P < 0.01). During oral glucose tolerance test (OGTT), god of 0 min was lower than that of DGMS P0. 000 ~ 0. 000 min. The correlation coefficient was 0. 989 (P < 0. 000). Conclusion there is a good correlation between whole blood glucose and DGMS in stable state. On the basis of steady-state, the glucose concentration monitored by DGMS increases with the rapid increase of blood glucose during OGTT or steamed bread meal tolerance test. The trend of change was similar to that of God, but the absolute value showed that the response of DGMS was slower than that of God, and the DGMS level was lower than that of God before 120 min after meal. However, there was no significant difference between DGMS and God blood glucose concentration from 120 min to 120 min. This result is made in the unsteady state, reflects the true blood sugar situation, and synthesizes the 5 points analysis, the final result is that DGMS and God have very good correlation.
