關(guān)節(jié)腔內(nèi)骨髓間充質(zhì)干細胞移植對類風濕性關(guān)節(jié)炎軟骨的保護和修復(fù)作用

【摘要】:背景:類風濕性關(guān)節(jié)炎總是伴隨著關(guān)節(jié)滑膜增生和不同程度的軟骨破壞。近來,骨髓間充質(zhì)干細胞(BMMSCs)吸引了越來越多的關(guān)注。除了軟骨分化能力,,它還表現(xiàn)出免疫抑制的特性,并已應(yīng)用在各種自身免疫疾病的治療中。以往關(guān)于BMMSCs對類風濕性關(guān)節(jié)炎的研究主要通過靜脈或腹腔注射來觀察其對免疫系統(tǒng)的影響。而對關(guān)節(jié)的炎癥反應(yīng)的抑制和防止軟骨的持續(xù)性破壞的結(jié)果不佳。 目的:應(yīng)用經(jīng)典的II型膠原誘導的大鼠關(guān)節(jié)炎模型進行脛骨平臺鉆孔后,將溫敏性水凝膠包裹的BMMSCs原位注射于關(guān)節(jié)腔內(nèi),來觀察其對軟骨的保護作用。此外,在卵清蛋白誘導的兔關(guān)節(jié)炎模型中制造膝關(guān)節(jié)骨軟骨缺損,將纖維蛋白凝膠包裹的BMMSCs原位移植,并評估其對全層軟骨缺損的修復(fù)效果。 結(jié)果:II型膠原誘導的大鼠關(guān)節(jié)炎模型中,間充質(zhì)干細胞移植可以下調(diào)外周血中CD4+和CD8+淋巴細胞的比例;并能使血清中的炎性細胞因子,如白細胞介素-1和腫瘤壞死因子α,和II型膠原抗體的表達;此外,細胞移植還能抑制抗原特異淋巴細胞的增殖和關(guān)節(jié)局部的炎性反應(yīng)、阻止關(guān)節(jié)軟骨的進一步破壞。以上結(jié)果表明干細胞的局部移植聯(lián)合微骨折術(shù)可以有效地發(fā)揮其免疫抑制能力,保護軟骨的完整性。此外,在卵清蛋白誘導的兔類風濕性關(guān)節(jié)炎模型中,纖維蛋白凝膠包載的間充質(zhì)干細胞在骨和軟骨下骨缺損處可以發(fā)揮軟骨修復(fù)的作用,并能改善關(guān)節(jié)局部的炎性環(huán)境,延緩軟骨的進一步破壞。 結(jié)論:間充質(zhì)干細胞的局部移植聯(lián)合微骨折術(shù)可以改善類風濕性關(guān)節(jié)炎的癥狀,誘導內(nèi)源性干細胞發(fā)揮免疫抑制能力,保護軟骨免于進一步的破壞,在骨和軟骨下骨缺損處可以起到修復(fù)軟骨缺損的作用。
[Abstract]:Background: rheumatoid arthritis is always associated with joint synovial hyperplasia and varying degrees of cartilage destruction. Recently, bone marrow mesenchymal stem cells (BMMSCs) attract more and more attention. In addition to cartilage differentiation, it has shown the characteristics of immunosuppression and has been used in the treatment of various autoimmune diseases. Previous BMMSCs pairs were used. The study of rheumatoid arthritis is mainly by intravenous or intraperitoneal injection to observe its effects on the immune system. The inhibition of the inflammatory response to the joints and the prevention of persistent destruction of the cartilage are poor.
Objective: to apply the classic II collagen induced rat arthritis model to the tibial plateau drilling, the thermosensitive hydrogel enwrapped BMMSCs was injected into the articular cavity to observe its protective effect on the cartilage. In addition, the osteochondral defect of the knee joint was made in the rabbit arthritis model induced by ovalbumin, and the fibrin gel was used. BMMSCs in situ was transplanted and the effect of repairing the full-thickness cartilage defect was evaluated.
Results: in the rat model of type II collagen induced arthritis, mesenchymal stem cell transplantation can reduce the proportion of CD4+ and CD8+ lymphocytes in peripheral blood, and the expression of inflammatory cytokines in the serum, such as interleukin -1 and tumor necrosis factor alpha, and II type collagen antibody. Moreover, cell transplantation can inhibit antigen specific drench. The proliferation of the cell and the local inflammatory reaction of the joint prevent the further destruction of the articular cartilage. The results show that the local transplantation of the stem cells combined with the micro fracture can effectively exert its immunosuppressive ability and protect the integrity of the cartilage. In addition, in the rabbit model of rheumatoid arthritis induced by ovalbumin, fibrin gel The entrapment of mesenchymal stem cells can play the role of cartilage repair at the bone and subchondral bone defects, and can improve the local inflammatory environment and delay the further damage of cartilage.
Conclusion: local transplantation of mesenchymal stem cells combined with micro fracture can improve the symptoms of rheumatoid arthritis, induce endogenous stem cells to play immunosuppressive ability, protect cartilage from further destruction, and play a role in repairing cartilage defects at the bone and subchondral bone defects.
