【摘要】：目的探討急性有機(jī)磷農(nóng)藥中毒患者血清及全血膽堿酯酶的相關(guān)性。方法選擇該院收治的有機(jī)磷農(nóng)藥中毒患者79例作為觀察組,另選取40例健康老年體檢者作為對(duì)照組。79例患者均使用復(fù)能劑治療,維持生命體征平穩(wěn)。抽取患者入院8h以?xún)?nèi)及第2、3、4、5、6、7、8天和出院前1d同時(shí)段的外周血檢測(cè)血清丁酰膽堿酯酶(BuChE)和全血乙酰膽堿酯酶(AChE)活力,同時(shí)進(jìn)行中毒指數(shù)評(píng)估。結(jié)果對(duì)照組的BuChE活性明顯高于輕、中、重度有機(jī)磷中毒患者(F=249.41,P0.01)。輕、中度有機(jī)磷中毒患者入院第5天,中毒指數(shù)、血清BuChE、全血AChE活力3項(xiàng)指標(biāo)改善明顯,相關(guān)性分析顯示中毒指數(shù)和全血AChE呈負(fù)相關(guān)(r=-0.95、-0.89,P0.01);中毒指數(shù)和血清BuChE呈負(fù)相關(guān)(r=-0.92、-0.90,P0.01);血清BuChE和全血AChE呈正相關(guān)(r=0.87、0.91,P0.01)。重度有機(jī)磷中毒患者入院后第8天,3項(xiàng)指標(biāo)改善明顯,相關(guān)性分析顯示中毒指數(shù)和全血AChE呈負(fù)相關(guān)(r=-0.91,P0.01);與血清BuChE呈負(fù)相關(guān)(r=-0.91,P0.01);血清BuChE和全血AChE呈正相關(guān)(r=0.88,P0.01)。結(jié)論血清BuChE活性可作為早期急性有機(jī)磷農(nóng)藥中毒診斷的敏感指標(biāo),治療后患者血清BuChE活力的恢復(fù)和臨床癥狀密切相關(guān)。
[Abstract]:Objective to study the correlation between serum and whole blood cholinesterase in patients with acute organophosphorus pesticide poisoning. Methods Seventy-nine patients with organophosphorus pesticide poisoning treated in our hospital were selected as observation group and 40 healthy aged people as control group. 79 patients were treated with reactives to maintain stable vital signs. The activities of serum butyrylcholinesterase (BuChE) and whole blood acetylcholinesterase (ache) were measured in peripheral blood of patients within 8 hours of admission and at the same time of 78th day after admission and 1 day before discharge, and the poisoning index was evaluated at the same time. Results the BuChE activity in the control group was significantly higher than that in the mild, moderate and severe organophosphorus poisoning patients. On the 5th day after admission to hospital, the poisoning index, serum Buchem and AChE activity in patients with mild and moderate organophosphorus poisoning were significantly improved. The correlation analysis showed that there was a negative correlation between the poisoning index and the whole blood AChE, a negative correlation between the poisoning index and the serum BuChE, and a positive correlation between the serum BuChE and the whole blood AChE. Three indexes of severe organophosphorus poisoning were significantly improved on the 8th day after admission. The correlation analysis showed that the poisoning index was negatively correlated with the whole blood AChE, the serum BuChE was negatively correlated with the serum BuChE, and the serum BuChE was positively correlated with the whole blood AChE. Conclusion Serum BuChE activity can be used as a sensitive index for early diagnosis of acute organophosphorus pesticide poisoning. The recovery of serum BuChE activity is closely related to clinical symptoms after treatment.
