【摘要】：目的觀察早期類風(fēng)濕性關(guān)節(jié)炎(RA)患者手腕關(guān)節(jié)超聲檢查結(jié)果。方法早期RA的30例患者(RA組),應(yīng)用高頻彩色多普勒超聲觀察腕關(guān)節(jié)、掌指關(guān)節(jié)及近端指間關(guān)節(jié)共660處,測量關(guān)節(jié)積液及滑膜厚度,觀察滑膜內(nèi)血流特點,手背及手掌側(cè)均行檢查,并與10例健康者(對照組)雙側(cè)相應(yīng)關(guān)節(jié)220處檢查結(jié)果進(jìn)行對照。結(jié)果RA組手背側(cè)、手掌側(cè)關(guān)節(jié)積液總檢出率分別為14%、26%,滑膜增生總檢出率分別為33%、11%,兩者比較,P均0.05。RA組146個關(guān)節(jié)增厚滑膜中可檢出血流信號,檢出率51%。結(jié)論類風(fēng)濕疾病累及手部小關(guān)節(jié)時,手背側(cè)超聲檢查更易檢出滑膜增生,手掌側(cè)更易檢出關(guān)節(jié)積液,手背及手掌側(cè)手腕關(guān)節(jié)均行檢查可以更全面了解病情。
[Abstract]:Objective to observe the results of wrist joint ultrasonography in patients with early rheumatoid arthritis (RA). Methods the wrist joint, metacarpophalangeal joint and proximal interphalangeal joint were observed by high frequency color Doppler ultrasound in 30 patients with early RA. The thickness of synovial fluid and synovial membrane were measured, and the characteristics of intrasynovial blood flow were observed. The dorsal and palmar sides of the hand were examined, and the results were compared with those of 10 healthy subjects (control group). Results in RA group, the total detectable rates of hand dorsal and palmar joint effusion were 1426g, and the total detectable rate of synovial hyperplasia were 331.The results were compared with that in 0.05.RA group (P < 0.05). The bleeding flow signals were detected in 146 joints thickened synovium in 0.05.RA group, and the detectable rate was 51%. Conclusion when rheumatoid diseases involve the facet joints of the hand, the synovial hyperplasia is more easily detected by ultrasonic examination on the dorsal side of the hand, and the effusion of the joint on the palmar side is easier to be detected. The condition of the hand can be understood more comprehensively by the examination of both the dorsal and the palmar wrist joints.
