血清趨化因子水平與新診斷2型糖尿病患者認知功能的相關(guān)性研究

【摘要】:目的糖尿病引起認知功能下降已成為研究熱點。文中探討新診斷2型糖尿病(T2DM)患者血清調(diào)節(jié)活化正常T細胞表達與分泌的趨化因子(RANTES)水平與認知功能障礙的關(guān)系,為臨床新診斷T2DM伴認知功能障礙的防治提供新的治療思路。方法回顧性分析2015年3月至2016年9月期間于唐山市工人醫(yī)院就診并首次確診的123例T2DM患者一般資料及臨床生化指標(biāo)。采用酶聯(lián)免疫吸附法(ELISA)測定血清RANTES水平,同時采用簡易智能精神狀態(tài)檢查量表(MMSE)和重復(fù)性成套神經(jīng)心理狀態(tài)測驗量表(RBANS)對所有患者的認知功能進行評定。最終依據(jù)MMSE評分將新診斷的T2DM患者分為T2DM非認知障礙組與T2DM認知障礙組。分析以上2組患者一般資料、血清RANTES水平及RBANS認知功能量表評分的差異。采用單因素相關(guān)及多元逐步回歸分析RANTES與基線資料及認知評分的相關(guān)性。結(jié)果 T2DM認知障礙組血清RANTES為(2.62±0.37)mmol/L,與T2DM非認知障礙組[(2.29±0.36)mmol/L]比較明顯升高(P0.05),T2DM認知障礙組即刻記憶、視覺廣度、注意力、延遲記憶評分及RBANS量表總分分別為(70.90±14.71)、(92.90±15.50)、(87.80±16.45)、(88.02±14.28)和(82.92±11.07),與T2DM非認知障礙組[分別為(85.28±13.97)、(104.18±12.69)、(101.51±12.94)、(96.42±10.30)、(95.84±9.94)]比較均明顯降低(P0.05);2組言語功能評分分別為(96.08±7.87)和(99.31±9.83),差異無統(tǒng)計學(xué)意(P=0.056);T2DM患者中RANTES與即刻記憶、視覺廣度、言語功能、延遲記憶評分及RBANS量表總分呈負相關(guān)(r值分別為-3.48、-2.35、-2.01、-3.02和-4.17)。結(jié)論血清RANTES水平與認知障礙存在顯著相關(guān)性。血清RANTES水平升高可作為監(jiān)測新診斷T2DM患者伴有認知功能障礙的重要指標(biāo)。
[Abstract]:Objective Diabetes mellitus (DM) induced cognitive impairment has become a hot research topic. To explore the relationship between the level of serum regulatory activated normal T cell expression and secreted chemokine (RANTES) and cognitive dysfunction in patients with newly diagnosed type 2 diabetes mellitus (T2DM), and to provide a new therapeutic approach for the prevention and treatment of newly diagnosed T2DM with cognitive dysfunction. Methods from March 2015 to September 2016, the general data and clinical biochemical indexes of 123 patients with T2DM who were first diagnosed in Tangshan Workers Hospital were analyzed retrospectively. The level of serum RANTES was measured by Elisa, and the cognitive function of all patients was evaluated by simple Mental State examination scale (MMSE) and repetitive Neuropsychological State Test scale (RBANS). According to the MMSE score, the newly diagnosed T2DM patients were divided into T2DM non-cognitive impairment group and T2DM cognitive impairment group. The general data, serum RANTES level and RBANS cognitive function scale scores of the two groups were analyzed. Single factor correlation and multiple stepwise regression analysis were used to analyze the correlation between RANTES and baseline data and cognitive score.
