【摘要】：目的探討孤兒核受體NR4A1在2型糖尿病(T2DM)患者外周血單個(gè)核細(xì)胞(PBMCs)的表達(dá)及意義。方法選取就診的34例健康受試者和30例初診T2DM患者,進(jìn)行體格檢查,包括身高、體重、血壓,計(jì)算體質(zhì)指數(shù)(BMI);實(shí)驗(yàn)室檢查:空腹血糖(FBG)、空腹胰島素(Fins)、三酰甘油(TG)、總膽固醇(TC)、低密度脂蛋白膽固醇(LDL-C)、高密度脂蛋白膽固醇(HDL-C)、超敏C反應(yīng)蛋白(hs-CRP),計(jì)算HOMA-IR。采集外周血2~3 m L,收集血漿,用于檢測(cè)游離脂肪酸(FFAs)、腫瘤壞死因子-α(TNF-α)、白細(xì)胞介素-6(IL-6),并采用密度梯度離心法獲取PBMCs,real-time PCR檢測(cè)NR4A1 mRNA在健康人群和初診T2DM患者PBMCs中分布情況,并對(duì)NR4A1 mRNA相對(duì)表達(dá)水平和HOMA-IR、FFAs、TNF-α、IL-6進(jìn)行相關(guān)性分析。結(jié)果兩組人群在年齡、性別、身高、血壓、血丙氨酸氨基轉(zhuǎn)移酶、血尿素氮、血肌酐方面,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);與健康受試者比較,初診T2DM患者的PBMCs的NR4A1 mRNA相對(duì)表達(dá)水平增高,此外T2DM患者的BMI、FBG、Fins、HOMAIR、TG、TC、LDL-C、hs-CRP、FFAs、TNF-α和IL-6水平亦明顯增高,血HDL-C水平則降低;T2DM患者的外周血白細(xì)胞計(jì)數(shù)及其分類計(jì)數(shù)與健康受試者差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);Pearson's相關(guān)分析顯示,NR4A1 mRNA相對(duì)表達(dá)水平與HOMA-IR(r=0.761,P0.01)、FFAs(r=0.560,P0.01)、TNF-α(r=0.697,P0.01)和IL-6(r=0.796,P0.01)均呈正相關(guān)。同時(shí),評(píng)價(jià)胰島素抵抗程度的HOMA-IR也與FFAs(r=0.513,P0.01)、TNF-α(r=0.728,P0.01)和IL-6(r=0.590,P0.01)呈正相關(guān)。此外,顯著正相關(guān)也存在于FFAs和TNF-α(r=0.475,P0.01)、IL-6(r=0.402,P0.01)之間。結(jié)論 NR4A1在初診T2DM患者PBMCs中表達(dá)增高,且與HOMA-IR、FFAs、TNF-α、和IL-6均呈正相關(guān)關(guān)系。
[Abstract]:Objective to investigate the expression and significance of orphan nuclear receptor (NR4A1) in peripheral blood mononuclear cells (PBMCs) from patients with type 2 diabetes mellitus (T2DM). Methods 34 healthy subjects and 30 newly diagnosed T2DM patients were selected for physical examination, including height, weight, blood pressure. Calculated body mass index (BMI); laboratory examination: fasting blood glucose FBGG, fasting insulin Finsn, triacylglycerol TGG, total cholesterol TCU, LDL-Cn, HDL-CU, HDL-CU, hypersensitive C-reactive protein hs-CRPU, HOMA-IRN were calculated in all patients with high density lipoprotein cholesterol (HDL-C), and hypersensitive C-reactive protein (HDL-CRPU). Peripheral blood samples were collected at 2mL, and plasma samples were collected for the detection of free fatty acid (FFA), tumor necrosis factor- 偽 (TNF- 偽), interleukin-6 (IL-6), and the distribution of NR4A1 mRNA in PBMCs of healthy population and newly diagnosed T2DM patients was detected by real-time PCR obtained by density gradient centrifugation. The correlation between the relative expression of NR4A1 mRNA and Homa-IRFAsT TNF- 偽-6 was analyzed. Results there was no significant difference in age, sex, height, blood pressure, serum alanine aminotransferase, blood urea nitrogen and serum creatinine between the two groups. In addition, the levels of IL-6 and TNF- 偽 in T2DM patients were also significantly higher than those in patients with T2DM, and the levels of LDL-Cnhs-CRP FFAsTNF- 偽 and TNF- 偽 were also significantly increased in patients with T2DM, and the levels of TNF- 偽 and TNF- 偽 were also significantly increased in patients with T2DM. There was no significant difference in peripheral blood white blood cell count and its classification between patients with T2DM and healthy subjects. The correlation analysis showed that the relative expression level of NR4A1 mRNA was positively correlated with that of HOMA-IRR 0.761P0.01FFASS 0.560P0.01TNF- 偽 TNF- 偽 0.796P0.01). At the same time, HOMA-IR for evaluating the degree of insulin resistance was also positively correlated with TNF- 偽 -0.728 (P0.01) and IL-6 (0.590) P0.01). In addition, a significant positive correlation was also found between FFAs and TNF- 偽 (0.475P0.01P0.01P0.01). Conclusion the expression of NR4A1 in PBMCs of newly diagnosed T2DM patients is increased, and it is positively correlated with HOMA-IRFFAsTNF- 偽 and IL-6.
