【摘要】：目的調(diào)查海南省地方性甲狀腺腫流行病學(xué)特點(diǎn),并分析其干預(yù)措施。方法采取流行病學(xué)的調(diào)查方法對(duì)海南省2010年~2015年間的1200例地方性甲狀腺腫患者的發(fā)病特點(diǎn)和發(fā)病趨勢(shì)以及臨床表現(xiàn),并依據(jù)流行病特點(diǎn)進(jìn)行制定干預(yù)措施。結(jié)果男性500例(比例41.7%),女性700例(58.3%),男性地方性甲狀腺腫發(fā)病率明顯的低于女性,數(shù)據(jù)比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。地方性甲狀腺腫患者的年齡分布比較廣泛,年齡段為16~20、21~30、31~40、41~50、51~60、61~70、71~77,構(gòu)成比分別為7.4%、10.1%、12.1%、14.1%、16.0%、17.2%、23.1%。2010年、2011年、2012年、2013年、2014年、2015年中地方性甲狀腺腫發(fā)生率分別為31.7%、22.8%、18.2%、14.3%、8.7%、4.3%。B超檢查地方性甲狀腺腫主要分型為彌漫性腫、結(jié)節(jié)性腫、甲狀腺瘤、膠質(zhì)性腫和甲狀腺癌,構(gòu)成比分別為54.3%、30.1%、9.0%、4.8%、1.8%。地方性甲狀腺腫臨床分度表現(xiàn)為Ⅰ度、Ⅱ度和Ⅲ度以及Ⅳ度,構(gòu)成比分別為91.3%、5.7%、2.1%、1.0%。地方性甲狀腺腫患者飲用水含碘量為86-1895μg/L。其中,鄉(xiāng)鎮(zhèn)地區(qū)、縣級(jí)地區(qū)、市級(jí)地區(qū)的飲用水含碘量分別為(1217.5±2311.7)、(756.3±99.6)、(174.7±21.6)μg/L,且市級(jí)地區(qū)飲用水含碘量明顯的低于鄉(xiāng)鎮(zhèn)地區(qū)、縣級(jí)地區(qū)飲用水含碘量,數(shù)據(jù)比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論海南省地方性甲狀腺腫患者流行病學(xué)特點(diǎn)相對(duì)比較明顯,其發(fā)病可能與患者的生活水平以及碘攝入量有著關(guān)聯(lián),可以依據(jù)臨床特征進(jìn)行制定干預(yù)措施,改善海南省地區(qū)地方性甲狀腺腫發(fā)病狀況。
[Abstract]:Objective to investigate the epidemiological characteristics of local goiter in Hainan Province, and to analyze its intervention measures. Methods the epidemiological characteristics and clinical manifestations of 1200 patients with endemic goiter in Hainan Province during ~2015 2010 were investigated by epidemiological investigation methods, and the intervention measures were made according to the epidemiological characteristics. There were 500 cases (41.7%) and 700 women (58.3%). The incidence of local goiter was significantly lower than that of women. The difference of data was statistically significant (P0.05). The age distribution of the patients with endemic goiter was 16~20,21~30,31~40,41~ 50,51~60,61~70,71~77, and the constituent ratio was 7.4%, 10.1%, 12.1%, 14.1%, 16, respectively. In%, 17.2%, 23.1%.2010, 2011, 2012, 2013, 2014, and 2015, the incidence of endemic goiter was 31.7%, 22.8%, 18.2%, 14.3%, 8.7%. Local goiter was mainly classified as diffuse swelling, nodular swollen, thyroid tumor, gelatinous swelling and thyroid cancer, and the constituent ratio was 54.3%, 30.1%, 9%, 4.8%, 1.8%., respectively. The clinical score of square goiter was I degree, II degree, grade III and IV degree, the ratio of the composition ratio was 91.3%, 5.7%, 2.1%. The iodine content of drinking water in the patients with endemic goiter was 86-1895 mu g/L., the iodine content of drinking water in the township, county and municipal areas was (1217.5 + 2311.7), (756.3 + 99.6) and (174.7 + 21.6) mu g/L, respectively. The iodine content in the drinking water in the municipal area is obviously lower than that in the township, and the iodine content in the drinking water in the county area is statistically significant (P0.05). Conclusion the epidemiological characteristics of the local goiter in Hainan province are relatively obvious, and the incidence of the patients may be related to the living level and iodine intake of the patients. Clinical features were formulated to improve the incidence of endemic goiter in Hainan province.
