【摘要】：目的分析痛風(fēng)性關(guān)節(jié)炎骨質(zhì)疏松患者血清瘦素(LEP)、骨保護(hù)素(OPG)與炎癥因子白細(xì)胞介素(IL-6)、腫瘤壞死因子α(TNF-α)相關(guān)性。方法選擇痛風(fēng)性關(guān)節(jié)炎患者48例為研究對(duì)象(GA組),其中伴有骨質(zhì)疏松19例(OP組),不伴有骨質(zhì)疏松29例(非OP組),另選擇健康體檢者45例為對(duì)照。以雙能X線骨密度吸收儀測(cè)定骨密度,ELISA檢測(cè)血清LEP、OPG及IL-6、TNF-α水平并分析相關(guān)性。結(jié)果 GA組患者骨質(zhì)疏松發(fā)生率39.6%明顯高于對(duì)照組的13.3%(P=0.004),各部位骨密度明顯低于對(duì)照組(P0.05)。與對(duì)照組及非OP組相比,OP組患者血清LEP水平明顯升高,OPG水平明顯降低,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。非OP組患者血清LEP水平明顯高于對(duì)照組(P0.05),但OPG水平差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。OP組患者血清IL-6、TNF-α水平明顯高于非OP組及對(duì)照組(P0.05),非OP組明顯高于對(duì)照組(P0.05)。OP組患者血清LEP與IL-6、TNF-α呈正相關(guān)(P0.05),血清OPG與IL-6、TNF-α呈負(fù)相關(guān)(P0.05)。結(jié)論痛風(fēng)性關(guān)節(jié)炎伴發(fā)骨質(zhì)疏松患者血清LEP、OPG呈異常變化,其與IL-6、TNF-α有一定相關(guān)性。
[Abstract]:Objective to analyze the relationship between serum leptin (leptin), osteoprotegerin (OPG) and inflammatory factor interleukin-6 (IL-6), tumor necrosis factor 偽 (TNF- 偽) in patients with gouty arthritis and osteoporosis. Methods A total of 48 patients with gouty arthritis were studied in GA group, including 19 patients with osteoporosis in op group, 29 patients without osteoporosis (non-op group) and 45 healthy persons as control. The serum levels of LEP- OPG and IL-6 TNF- 偽 were measured by dual-energy X-ray absorptiometry and the correlation was analyzed. Results the incidence of osteoporosis in GA group was significantly higher than that in control group (39.6%), and the bone mineral density (BMD) in all parts of GA group was significantly lower than that in control group (P 0.05). Compared with the control group and the non-op group, the serum LEP level in the op group was significantly higher than that in the control group and the non-op group, and the difference was statistically significant (P 0.05). The level of serum LEP in non-op group was significantly higher than that in control group (P0.05), but there was no significant difference in OPG level. The level of IL-6TNF- 偽 in patients with non-op group was significantly higher than that in non-op group and control group (P0.05A). The level of serum LEP and IL-6 TNF- 偽 in non-op group was significantly higher than that in control group (P0.05). The level of IL-6 TNF- 偽 in op group was significantly higher than that in control group (P 0.05). There was a positive correlation between serum OPG and IL-6 TNF- 偽, and a negative correlation between serum OPG and IL-6 TNF- 偽. Conclusion the serum levels of LEP- OPG in patients with gouty arthritis complicated with osteoporosis are abnormal, which is related to IL-6 TNF- 偽.
