呼吸功能訓(xùn)練對無創(chuàng)正壓通氣治療重度急性有機氟中毒患者康復(fù)期肺功能影響

【摘要】:目的分析研究對接受無創(chuàng)正壓通氣治療的重度急性有機氟中毒患者實施呼吸功能訓(xùn)練對其肺功能的影響。方法62例重度急性有機氟中毒患者,隨機分為觀察組與對照組,各31例。兩組患者均進行無創(chuàng)正壓通氣治療,對照組僅進行日常鍛煉,觀察組進行縮唇-腹式呼吸及人工阻力呼吸訓(xùn)練,對比觀察兩組患者治療效果、肺通氣功能變化情況及治療前后血清α-AT1、SP-D、NE、TGF-β1和生活質(zhì)量變化情況。結(jié)果觀察組治療總有效率為93.55%,高于對照組的74.19%,且FEV1、FEV1與FVC比值、VC、DLco及MVV水平均明顯優(yōu)于對照組,差異有統(tǒng)計學(xué)意義(P0.05);治療前兩組血清中α-AT1、SP-D、NE、TGF-β1水平及生活質(zhì)量評分比較,差異無統(tǒng)計學(xué)意義(P0.05);治療后,觀察組各項指標及生活質(zhì)量評分均明顯優(yōu)于對照組,差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論重度急性有機氟中毒患者在無創(chuàng)正壓通氣治療基礎(chǔ)上采用呼吸功能訓(xùn)練,可有效改善血清各項指標,稀釋體內(nèi)毒性,恢復(fù)患者肺部功能,對提高其治療效果和生活質(zhì)量具有重要意義,值得推廣運用。
[Abstract]:Objective to study the effects of respiratory function training on lung function in patients with severe acute organic fluorosis treated by non-invasive positive pressure ventilation. Methods Sixty-two patients with severe acute organic fluorosis were randomly divided into observation group and control group, 31 cases in each group. The patients in both groups were treated with non-invasive positive pressure ventilation, the control group was only given daily exercise, the observation group was trained with labial and ventral breathing and artificial resistance breathing, and the therapeutic effect was compared between the two groups. The changes of pulmonary ventilation function and the changes of serum 偽 -AT1, SP-DX, TGF- 尾 1 and quality of life before and after treatment. Results the total effective rate of treatment in the observation group was 93.55, which was higher than that in the control group (74.19%), and the ratio of FEV1FEV1 to FVC was significantly higher than that in the control group (P 0.05). After treatment, the indexes and quality of life scores in the observation group were significantly better than those in the control group, and the difference was statistically significant (P 0.05). Conclusion Respiratory function training on the basis of noninvasive positive pressure ventilation in patients with severe acute organic fluorosis can effectively improve the indexes of serum, dilute the toxicity in vivo and restore the pulmonary function of the patients. It is of great significance to improve the therapeutic effect and quality of life, and is worth popularizing.
