【摘要】：目的研究近3年間石河子社區(qū)管理2型糖尿病(T2DM)患者降糖方案構(gòu)成比和臨床結(jié)局變化情況。方法采用整群抽樣的方法于2012年11月從石河子的13個(gè)社區(qū)隨機(jī)抽取符合標(biāo)準(zhǔn)的T2DM患者300例,進(jìn)行為期3年的隨訪觀察,收集一般人口學(xué)信息、治療方案、臨床結(jié)局等資料。結(jié)果與2012年比較,2015年口服藥物組患者比例從63.5%降低到51.0%,胰島素治療組從30.8%增加到41.8%,其中單藥治療組的比例明顯下降(χ~2=8.77,P0.05),胰島素聯(lián)合口服藥物組的比例明顯上升(χ~2=-10.27,P0.01),用藥不良反應(yīng)發(fā)生率從16.8%增高至24.5%;2015年血糖指標(biāo)及達(dá)標(biāo)率較2012年無明顯變化,2015年血糖達(dá)標(biāo)率按(1)標(biāo)準(zhǔn)計(jì)算為41.2%,較2012年減少了4.0%,按(2)標(biāo)準(zhǔn)計(jì)算為52.9%,較2012年增加了1.4%;2015年糖尿病合并癥和并發(fā)癥較2012年明顯增加。結(jié)論 (1)石河子社區(qū)T2DM患者治療以口服藥物治療為主,胰島素的使用率有所上升;(2)石河子社區(qū)在T2DM患者血糖達(dá)標(biāo)、用藥安全性、延緩其并發(fā)癥及合并癥進(jìn)展等方面有待提高。
[Abstract]:Objective to study the changes of the composition ratio of hypoglycemic regimen and clinical outcome in patients with type 2 diabetes mellitus (T2DM) managed in Shihezi community in recent three years. Methods three hundred T2DM patients were randomly selected from 13 communities in Shihezi by cluster sampling in November, 2012. The data of general demographic information, treatment scheme and clinical outcome were collected for a period of 3 years. Results compared with 2012, the proportion of patients in oral drug group decreased from 63.5% to 51.0% in 2015, and that in insulin treatment group increased from 30.8% to 41.8%, in which the proportion of single drug group decreased significantly (蠂 ~ (2 +) 8.77) P 0.05, and the proportion of insulin combined oral drug group increased significantly (蠂 ~ (2) -10.27 P _ (0.01), P < 0.05). The incidence of adverse drug reactions increased from 16.8% to 24.5%, and there was no significant change in the blood glucose index and the rate of reaching the standard in 2015 compared with 2012. The 2015 blood glucose reached standard rate was 41.2%, which was 4.0% lower than that in 2012, and 52.9% higher than that in 2012, which was 1.4 cents higher than that in 2012. Urinary complications and complications increased significantly compared with 2012. Conclusion 1) Oral drug therapy is the main treatment for T2DM patients in Shihezi community, and the utilization rate of insulin has increased. 2) in Shihezi community, blood glucose standard, drug safety, complications and complications in Shihezi community need to be improved.
