【摘要】：目的:評(píng)估3種雙膦酸鹽預(yù)防中國(guó)婦女絕經(jīng)后骨質(zhì)疏松骨折的有效性、安全性和經(jīng)濟(jì)性。方法:計(jì)算機(jī)檢索國(guó)內(nèi)外常用文獻(xiàn)數(shù)據(jù)庫(kù)中使用阿侖膦酸、利塞膦酸、唑來(lái)膦酸預(yù)防中國(guó)婦女絕經(jīng)后骨質(zhì)疏松骨折的隨機(jī)對(duì)照試驗(yàn)(RCT),提取資料后進(jìn)行Meta分析,并建立決策樹(shù)模型進(jìn)行成本-效果分析。結(jié)果:Meta分析結(jié)果顯示,與單用鈣劑/維生素D_3方案相比,聯(lián)用雙膦酸鹽能顯著減少中國(guó)婦女絕經(jīng)后骨質(zhì)疏松的椎體骨折[RR0.50,95%CI(0.33,0.74)]和非椎體骨折[RR0.36,95%CI(0.20,0.65)]的風(fēng)險(xiǎn),差異有統(tǒng)計(jì)學(xué)意義(P0.01)。亞組分析顯示,唑來(lái)膦酸能顯著減少中國(guó)婦女絕經(jīng)后骨質(zhì)疏松椎體骨折[RR0.44,95%CI(0.25,0.77)]風(fēng)險(xiǎn),阿侖膦酸和利塞膦酸亦表現(xiàn)出減少絕經(jīng)后骨質(zhì)疏松非椎體骨折的風(fēng)險(xiǎn)的優(yōu)勢(shì)[RR0.16,95%CI(0.04,0.72)和RR0.28,95%CI(0.10,0.80)]。安全性方面,加用雙膦酸鹽期間不良事件的發(fā)生率與對(duì)照組比較差異無(wú)統(tǒng)計(jì)學(xué)意義。成本-效果分析顯示,聯(lián)用雙膦酸鹽較單用鈣劑和維生素D_3方案經(jīng)濟(jì)性更優(yōu),阿侖膦酸聯(lián)合鈣劑/維生素D3方案為經(jīng)濟(jì)性最優(yōu)方案。結(jié)論:聯(lián)用雙膦酸鹽(尤其是阿侖膦酸)較單用鈣劑/維生素D_3方案能顯著地減少中國(guó)婦女絕經(jīng)后骨質(zhì)疏松骨折的風(fēng)險(xiǎn),安全性較好,經(jīng)濟(jì)性更優(yōu)。
[Abstract]:Objective: to evaluate the efficacy, safety and economy of three bisphosphonates in the prevention of postmenopausal osteoporosis in Chinese women.Methods: a randomized controlled trial of alendronic acid, Lise phosphonic acid and zoledronic acid to prevent postmenopausal osteoporosis fracture in Chinese women was searched by computer. The data were extracted and analyzed by Meta.A decision tree model is established for cost-effect analysis.Results compared with calcium / vitamin D3 regimen alone, the combined use of bisphosphonates significantly reduced the risk of postmenopausal osteoporosis vertebral fracture [RR0.50C95CI0.33 / 0.74] and non-vertebral fracture (RR0.3695CI0.200.65). The difference was statistically significant (P 0.01).Subgroup analysis showed that zoledronic acid significantly reduced the risk of postmenopausal osteoporotic vertebral fracture in Chinese women [RR0.44-95CI0.250.77]. Alendronic acid and Lise phosphonic acid also showed an advantage in reducing the risk of nonvertebral fracture in postmenopausal osteoporosis (RR0.1695CI0.040.72) and RR0.2895CII 0.10 / 0.80].In terms of safety, the incidence of adverse events during the addition of bisphosphonates was not significantly different from that in the control group.Cost-effect analysis showed that the combined use of bisphosphonates was more economical than that of calcium and vitamin D3 alone, and alendronic acid combined with calcium / vitamin D3 was the best.Conclusion: the combination of bisphosphonates (especially alendronic acid) can significantly reduce the risk of postmenopausal osteoporosis fracture in Chinese women compared with calcium / vitamin D _ 3 alone. It is safe and economical.
