【摘要】：目的:觀察疏肝健脾消癭湯合優(yōu)甲樂治療肝郁脾虛型橋本氏甲狀腺炎合并甲減的臨床療效,并結(jié)合現(xiàn)代醫(yī)學(xué)深入探索中藥的作用機(jī)制,從而突出中醫(yī)藥的優(yōu)勢(shì)。方法:將臨床上診斷為橋本氏甲狀腺炎合并甲減肝郁脾虛型的60例患者隨機(jī)分為兩組:治療組(B組)、對(duì)照組(A組),B組30例,給予疏肝健脾消癭湯合優(yōu)甲樂治療;A組30例,給予優(yōu)甲樂治療,4周為一療程,連用12周。所有患者治療前、后測(cè)定:(1)甲狀腺激素水平及TPOAb、TGAb水平;(2)甲狀腺左右葉及峽部厚度;(3)CD4(10)、CD8(10)、CD4(10)/CD8(10)水平;(4)中醫(yī)證候積分及綜合療效評(píng)定;(5)血常規(guī)、肝功、腎功等安全性指標(biāo)。結(jié)果:兩組治療前比較,年齡、性別、病程、甲狀腺激素水平、TPOAb、TGAb滴度水平、甲狀腺左右葉及峽部厚度、CD4(10)、CD8(10)、CD4(10)/CD8(10)水平及中醫(yī)證候積分等方面無明顯差異,P(29)0.05,具有可比性;治療后比較:兩組綜合療效比較,B組總有效率96.7%;A組總有效率83.3%,B組總有效率明顯高于A組,P0.05;兩組癥狀積分均較治療前有所降低,B組療效優(yōu)于A組,P0.05;B組在改善甲狀腺激素(FT4、TSH)水平,TPOAb、TGAb滴度水平,甲狀腺左右葉及峽部厚度,CD4(10)、CD4(10)/CD8(10)水平方面明顯優(yōu)于A組,P0.05;安全性指標(biāo)較治療前均未見明顯變化。結(jié)論:疏肝健脾消癭湯合優(yōu)甲樂治療橋本氏甲狀腺炎合并甲減療效肯定,值得進(jìn)一步深入探討。
[Abstract]:Objective: to observe the clinical effect of Shugan Jianpi Xiaoying decoction and Youjia Le in treating Hashimoto's thyroiditis combined with hypothyroidism in the type of liver stagnation and spleen deficiency, and to explore the mechanism of traditional Chinese medicine in order to highlight the advantages of traditional Chinese medicine.Methods: sixty patients with Hashimoto's thyroiditis combined with hypothyroidism liver depression and spleen deficiency were randomly divided into two groups: treatment group B group, control group A group (n = 30) and group A (n = 30) treated with Shugan Jianpi Xiaoying decoction and you Jia Le group (n = 30).A course of treatment was given for 4 weeks and continued for 12 weeks.Before and after treatment, the levels of thyroid hormone and TPOAbTGAb were measured before and after treatment. (2) the thickness of the left and right lobes and isthmus of the thyroid gland was determined.Results: there was no significant difference between the two groups in age, sex, course of disease, thyroid hormone level and TPOAb TGAb titer, the thickness of left and right thyroid lobe and isthmus and the level of CD410 / CD810 / CD810 and the score of TCM syndromes (P290.05).Comparison after treatment: the total effective rate of group B was 96. 7% and the total effective rate of group B was significantly higher than that of group A (83.33%), and the total effective rate of group B was significantly higher than that of group A (P 0. 05%), and the symptom score of group B was lower than that of group B (P 0. 05%) before treatment, and the total effective rate of group B was better than that of group B (group B) in improving thyroid gland.The titer of TPOAbAb TGAb,The thickness of left and right lobe and isthmus of thyroid gland was significantly better than that of group A (P 0.05), and there was no significant change in safety index.Conclusion: the curative effect of Shugan Jianpi Xiaoying decoction combined with Youjia Le in treating Hashimoto's thyroiditis with hypothyroidism is definite and worthy of further discussion.
