【摘要】：目的評(píng)估原發(fā)性干燥綜合征(primary Sjogren's syndrome, pSS)患者軀體癥狀分布情況及外周血NKT (Natural killer T-cells)細(xì)胞比例及IFN-y分泌變化；探索NKT細(xì)胞在pSS中的調(diào)節(jié)機(jī)制。方法通過軀體癥狀群量表-15(Patient-Health-Questionnaire-15, PHQ-15)對(duì)pSS患者和健康對(duì)照者的軀體癥狀發(fā)生及分布情況進(jìn)行評(píng)估,并通過視覺模擬評(píng)分(visual analogue scale, VAS)了解患者生活質(zhì)量受疾病影響程度。使用流式細(xì)胞儀檢測(cè)pSS患者及健康對(duì)照者外周血NKT細(xì)胞比例及各亞群變化。使用a-半乳糖神經(jīng)酰胺體外刺激擴(kuò)增NKT細(xì)胞,評(píng)估患者NKT細(xì)胞增殖能力。對(duì)比血清、細(xì)胞培養(yǎng)液上清及NKT細(xì)胞胞內(nèi)IFN-γ分泌變化。結(jié)果pSS患者軀體癥狀分布與健康人一致,但發(fā)生率高于健康人,軀體障礙等級(jí)也較健康人高。疲勞是pSS患者中最為常見的軀體癥狀。pSS患者外周血NKT細(xì)胞比例顯著低于健康人,CD4+CD8-NKT亞群最顯著。體外刺激擴(kuò)增NKT細(xì)胞后IFN-γ分泌水平較健康人增強(qiáng)。結(jié)論pSS患者軀體癥狀發(fā)生率顯著高于健康人,軀體障礙程度較健康人嚴(yán)重。pSS患者外周血NKT細(xì)胞比例降低。pSS患者NKT細(xì)胞IFN-γ合成分泌能力增強(qiáng),Th1/Th2平衡受影響。
[Abstract]:Objective to evaluate the distribution of somatic symptoms in patients with primary Sjogren's syndrome (PSS), the percentage of NKT Natural killer T-cellsand the changes of IFN-y secretion, and to explore the regulatory mechanism of NKT cells in pSS.Methods the occurrence and distribution of somatic symptoms were evaluated by Patient-Health-Questionnaire-15 (PHQ-15) in patients with pSS and healthy controls.The percentage and subsets of NKT cells in peripheral blood of pSS patients and healthy controls were detected by flow cytometry.A-galactose-ceramide was used to stimulate and amplify NKT cells in vitro to evaluate the proliferative ability of NKT cells in patients.The changes of IFN- 緯 secretion in serum, supernatant of cell culture medium and NKT cells were compared.Results the distribution of somatic symptoms in patients with pSS was consistent with that in healthy persons, but the incidence rate was higher than that in healthy persons, and the grade of somatic disorders was higher than that of healthy people.Fatigue is the most common somatic symptom. The percentage of peripheral blood NKT cells in patients with PSS is significantly lower than that in healthy people.The level of IFN- 緯 secretion was increased after stimulation and amplification of NKT cells in vitro.Conclusion the incidence of somatic symptoms in patients with pSS is significantly higher than that in healthy persons. The degree of somatic disorders is significantly lower than that in patients with PSS. The proportion of NKT cells in peripheral blood of patients with PSS is lower. The ability of IFN- 緯 synthesis and secretion of NKT cells in patients with PSS is increased and the balance of Th1 / Th2 is affected.
