【摘要】：目的探討益腎補(bǔ)骨湯聯(lián)合椎體成形術(shù)治療老年骨質(zhì)疏松性胸腰椎壓縮骨折的臨床效果及對(duì)生存質(zhì)量的影響。方法選擇該院接受治療的100例老年骨質(zhì)疏松性胸腰椎壓縮骨折患者,采取計(jì)算機(jī)單盲分組法隨機(jī)分為兩組,對(duì)照組50例采取椎體成形術(shù)治療,觀察組50例采取椎體成形術(shù)聯(lián)合益腎補(bǔ)骨湯治療,比較兩組治療效果、疼痛評(píng)分、Oswestry功能障礙指數(shù)(ODI)、傷椎前后緣高度、后凸角度、骨密度、生存質(zhì)量評(píng)分、再次骨折發(fā)生率。結(jié)果觀察組治療優(yōu)良率高于對(duì)照組(P0.05)。手術(shù)后3個(gè)月,兩組疼痛評(píng)分、ODI明顯降低,傷椎前后緣高度、骨密度明顯增高,后凸角度明顯減小,生存質(zhì)量評(píng)分明顯增高,且觀察組均明顯優(yōu)于對(duì)照組(均P0.05)。隨訪(fǎng)1年發(fā)現(xiàn),觀察組的再次骨折發(fā)生率明顯低于對(duì)照組(P0.05)。結(jié)論椎體成形術(shù)聯(lián)合益腎補(bǔ)骨湯治療老年骨質(zhì)疏松性胸腰椎壓縮骨折具有顯著的臨床效果,可有效緩解骨折疼痛,恢復(fù)傷椎高度和椎體后凸角度,改善胸腰椎活動(dòng)功能,還能有效避免再次發(fā)生骨折,進(jìn)而提高患者的生存質(zhì)量。
[Abstract]:Objective to investigate the clinical effect of Yishen Bugu decoction combined with vertebroplasty in the treatment of osteoporotic thoracolumbar compression fracture and its effect on quality of life.Methods 100 elderly patients with osteoporotic thoracolumbar compression fractures were randomly divided into two groups, 50 patients in the control group were treated with vertebroplasty.In the observation group, 50 cases were treated with vertebroplasty combined with Yishen Bugu decoction. The therapeutic effects were compared between the two groups. Pain score and Oswestry dysfunction index (Oswestry dysfunction index), height of anterior and posterior edge of injured vertebrae, kyphoid angle, bone mineral density, quality of life score and the incidence of refracture were compared between the two groups.Results the excellent and good rate of treatment in the observation group was higher than that in the control group (P 0.05).At 3 months after operation, the pain score and ODI of the two groups were significantly decreased, the height of the anterior and posterior edge of the injured vertebrae, bone mineral density, kyphosis angle and quality of life were significantly increased, and the observation group was significantly better than the control group (all P 0.05).It was found that the incidence of refracture in the observation group was significantly lower than that in the control group for one year.Conclusion Vertebroplasty combined with Yishen Bu decoction in the treatment of senile osteoporotic thoracolumbar vertebral compression fracture has significant clinical effect, which can effectively relieve fracture pain, recover the height of injured vertebrae and kyphosis angle, and improve the function of thoracolumbar vertebrae.Can also effectively avoid the recurrence of fracture, and then improve the quality of life of patients.
