【摘要】：目的分析云南四個獨有少數(shù)民族糖尿病患病、知曉、治療和自我管理的社會經(jīng)濟影響因素。方法對云南省≥35歲的納西族、傈僳族、傣族和景頗族村民,采用多階段分層抽樣方法抽取5 600人進行問卷調(diào)查和體檢。應(yīng)用多水平模型分析影響糖尿病患病和管理的社會經(jīng)濟因素。結(jié)果糖尿病的總體患病率、知曉率、治療率和控制率分別為4.7%、63.7%、48.1%和24.8%;糖尿病病人采取降糖措施率、遵醫(yī)囑用藥率和監(jiān)測血糖率分別為91.0%、68.9%和36.5%。教育水平和家庭財產(chǎn)越高以及醫(yī)療服務(wù)可及性越好的糖尿病病人,其治療率越高。居住在收入越高鄉(xiāng)鎮(zhèn)者患糖尿病的風險越大,患病后治療的可能性也越高。教育水平越高、醫(yī)療服務(wù)可及性越好及居住在高收入鄉(xiāng)鎮(zhèn)者更易遵醫(yī)囑用藥;教育水平和家庭財產(chǎn)越高以及居住在教育水平較高鄉(xiāng)鎮(zhèn)者監(jiān)測血糖率越高。在高收入鄉(xiāng)鎮(zhèn),其糖尿病病人采取降糖措施的可能性也越大。結(jié)論云南獨有少數(shù)民族地區(qū)糖尿病的預防和干預工作應(yīng)同時考慮個體和鄉(xiāng)級社會經(jīng)濟影響因素。
[Abstract]:Objective to analyze the social and economic factors affecting diabetes prevalence, knowledge, treatment and self-management in four unique minority nationalities in Yunnan Province. Methods Naxi, Lisu, Dai and Jingpo villagers aged more than 35 years old in Yunnan Province were studied. A multi-stage stratified sampling method was used to collect 5,600 people for questionnaire and physical examination. The social and economic factors affecting the prevalence and management of diabetes mellitus were analyzed by multi-level model. Results the overall prevalence rate and awareness rate of diabetes mellitus were analyzed. The rates of treatment and control were 48.1% and 24.8%, respectively; the rate of hypoglycemic measures, the rate of prescribed medication and the rate of monitoring blood sugar were 91.0% and 36.5%, respectively. The higher the level of education and family property, the better the availability of medical services. The higher the treatment rate is, the greater the risk of diabetes is, and the higher the possibility of treatment is, the higher the education level is, the better the medical service is, and the more easily people living in the high-income villages and towns are more likely to follow the doctor's advice. The higher the level of education and family property, and the higher the blood sugar rate of those living in the higher education level township, the higher the blood sugar rate in the high income township. Conclusion the prevention and intervention of diabetes in the unique minority areas of Yunnan should consider both individual and township social and economic influence factors.
