【摘要】：目的探討老年2型糖尿病(T2DM)患者血尿酸(SUA)水平與代謝綜合征(MS)的相關(guān)性。方法選取齊齊哈爾醫(yī)學(xué)院附屬第三醫(yī)院2014年1月至2016年12月老年內(nèi)科就診的老年T2DM患者,調(diào)查其基本情況,并測(cè)定相關(guān)人體參數(shù)及生化指標(biāo),分析患者SUA水平與MS的相關(guān)性。結(jié)果老年T2DM患者高尿酸血癥患病率22.83%,MS患病率53.67%;高尿酸血癥組與正常尿酸組比較,性別(男性)構(gòu)成、體重指數(shù)(BMI)、腰臀比、空腹血糖(FPG)、餐后2 h血糖、糖化血紅蛋白(Hb A1c)、空腹胰島素(FPIN)、高血壓患病率、收縮壓(SBP)、總膽固醇(TC)、甘油三酯(TG)、高密度脂蛋白膽固醇(HDL-C)、SUA、MS患病率差異有統(tǒng)計(jì)學(xué)意義(均P0.05);經(jīng)Spearman相關(guān)分析發(fā)現(xiàn),老年T2DM患者,SUA水平與FPG、FPIN、SBP、TC、TG呈正相關(guān),與HDL-C呈負(fù)相關(guān)(均P0.05);MSⅠ、Ⅱ、Ⅲ、Ⅳ組SUA水平逐漸升高(P0.05);隨SUA水平升高,MS發(fā)病率逐漸增高(P0.05);經(jīng)Logistic回歸分析顯示,與SUA265.81μmol/L比較,老年T2DM患者265.81~319.39μmol/L、319.38~376.01μmol/L、376.01μmol/L SUA區(qū)間MS發(fā)病風(fēng)險(xiǎn)分別為2.077、3.232、5.414(均P0.05)。ROC分析,SUA水平300μmol/L可作為預(yù)測(cè)診斷參考,其曲線下面積(AUC)為0.620 9。結(jié)論老年T2DM患者SUA水平與MS密切相關(guān),且SUA水平高者M(jìn)S的發(fā)病風(fēng)險(xiǎn)較高。
[Abstract]:Objective to investigate the correlation between serum uric acid (UAA) level and metabolic syndrome (MS) in elderly patients with type 2 diabetes mellitus (T2DM). Methods elderly patients with T2DM from January 2014 to December 2016 were selected from the third affiliated Hospital of Qiqihar Medical College. The results showed that the prevalence rate of hyperuricemia in elderly patients with T2DM was 22.83% and the prevalence rate of MS was 53.67%, and that of hyperuricemia group was higher than that of normal uric acid group, and that of hyperuricemia group was higher than that of normal uric acid group, and that of hyperuricemia group was higher than that of normal uric acid group. Sex (male) composition, body mass index (BMI), waist to hip ratio (WHR), fasting blood glucose (FBG), 2 h postprandial blood glucose, glycosylated hemoglobin (HbHb A1cN), fasting insulin (FFP), prevalence of hypertension. There were significant differences in the prevalence of SBP, TC, TG, HDL-CU, HDL-CU and SUAMS in the elderly patients with T2DM by Spearman correlation analysis, it was found that there was a positive correlation between the SUA level in the elderly patients with T2DM and SBPTC TG, and a negative correlation between them and HDL-C (all P0.05% MS 鈪，
