住院老年患者骨骼肌減少與骨密度的關系

【摘要】:目的:探討住院老年人骨骼肌質(zhì)量、骨骼肌指數(shù)與骨密度的關系,骨骼肌減少、骨質(zhì)疏松和脆性骨折的關系,為老年人骨質(zhì)疏松及脆性骨折的臨床防治工作開拓新的思路。方法:連續(xù)選取重慶醫(yī)科大學附屬第一醫(yī)院住院老年患者562例,測量身高、體重,計算體重指數(shù)(BMI),利用雙能X線骨密度儀(DXA)測量不同部位的骨密度(BMD)和T值、四肢骨骼肌質(zhì)量(ASMM)、骨骼肌指數(shù)(RSMI)、脂肪百分比(fat%),用Pearson相關性分析法研究各變量之間的關系,logistic回歸法研究骨骼肌減少、骨質(zhì)疏松、脆性骨折的關系。結(jié)果:不同性別的老年人RSMI均與BMD顯著正相關(腰椎:男性r=0.203,P0.01,女性r=0.165,P0.01;髖部:男性r=0.253,P0.01,女性r=0.225,P0.01),且均與BMI有高度的正相關性(男性r=0.777,P0.01;女性r=0.791,P0.01),BMD亦與BMI呈顯著正相關(腰椎:男性r=0.319,P0.01,女性r=0.236,P0.01;髖部:男性r=0.199,P0.01,女性r=0.211,P0.01)。老年女性骨質(zhì)疏松和脆性骨折的患病率明顯高于男性,而老年男性骨骼肌減少的檢出率則顯著高于女性。老年男性中骨骼肌減少與骨質(zhì)疏松存在明顯的相關性(β=0=0.774, P=0.002, OR=2.168),而在老年女性中二者無明顯相關(P0.05);老年總?cè)后w脆性骨折與骨質(zhì)疏松顯著相關,且與骨骼肌減少顯著相關(P0.05=。結(jié)論:老年人群中,骨骼和肌肉聯(lián)系密切;骨質(zhì)疏松與骨骼肌減少存在性別差異:老年男性中二者存在明顯的正相關性,而老年女性中二者并無顯著相關;脆性骨折與骨質(zhì)疏松及骨骼肌減少均密切相關,增加肌肉量對老年人群骨質(zhì)疏松及其骨折的防治有重要意義。
[Abstract]:Objective: to investigate the relationship between skeletal muscle mass, skeletal muscle index and bone mineral density (BMD), skeletal muscle reduction, osteoporosis and brittle fracture in elderly inpatients. Methods: 562 elderly patients in the first affiliated Hospital of Chongqing Medical University were selected to measure their height and weight. Body mass index (BMI) was calculated and BMD and T values were measured by dual energy X-ray absorptiometry (DXA). Skeletal muscle mass, skeletal muscle index and fat percentage were measured by Pearson correlation analysis and logistic regression. Results: there was a significant positive correlation between RSMI and BMD in elderly patients of different genders (lumbar vertebra: male: r = 0.203, P = 0.165, P = 0.01, hip: male, r = 0.253, P = 0.01, female, r = 0.225, P 0.01, r = 0.777, P 0.01; female, n = 0.791a, P 0.01, P = 0.01, respectively). There was significant positive correlation (lumbar vertebrae: male = 0.319g / P 0.01, female: r = 0.236P 0.01; hip: male: r = 0.199 / P 0.01; female: r = 0.211P 0.01. the prevalence of osteoporosis and brittle fracture in elderly women was significantly higher than that in male). The prevalence of skeletal muscle reduction in the elderly was significantly higher than that in the female. There was a significant correlation between skeletal myopenia and osteoporosis in the aged men (尾 -0. 774, P0. 002, OR2. 168), but there was no significant correlation between the two in the elderly women, and there was no significant correlation between them in the elderly population (P 0. 05). Fracture was significantly associated with osteoporosis, Conclusion: there is a close relationship between bone and muscle in the elderly, and there is a gender difference between osteoporosis and skeletal muscle reduction, and there is a significant positive correlation between them in elderly men. But there was no significant correlation between them in elderly women, brittle fracture was closely related to osteoporosis and skeletal muscle reduction, increasing muscle mass was of great significance for the prevention and treatment of osteoporosis and fracture in elderly people.
